Description

Imperfect Heart is a podcast designed for, hosted by and recorded with those affected by or involved with the care and support for those with the birth defect of a Myocardial Bridge. Interviews with healthcare professionals, support professionals and those navigating their way through the often challenging process of diagnosis and treatment are the focus of the content. It’s their stories that are meant to inform, educate and even entertain in an effort to insure hope for the listener that what they are experiencing is real, that they are not alone and that there is hope for relief from the varied symptoms of a Myocardial Bridge.

The host, Jeff Holden, has his own story to tell from symptom to diagnosis to treatment thus giving him insight and empathy as well as experience in the process of advocating for the patient. The program also seeks to make the healthcare industry more aware of the real symptoms of Myocardial Bridges with an aim to eliminate the gaslighting that is so prevalent around this defect and directly address the reality that Myocardial Bridges may be responsible for many more deaths from sudden cardiac arrest than previously thought. Early diagnosis and treatment could not only improve but may actually save lives when properly applied. More information can be found on the podcast website, www.myimperfectheart.com.

This may be one of those episodes that could be considered controversial and I hope it does prompt questions. Dr. Christopher Kwon is a highly experienced cardiac surgeon whose practice in Lake Havasu, Arizona has brought him face-to-face with a growing number of patients living with symptomatic Myocardial Bridges. What made this conversation so compelling is Dr. Kwon’s dual approach—he doesn’t just perform the standard unroofing procedure. He also chooses to bypass the artery in the same operation, giving his patients the greatest chance for long-term relief, in his expert opinion. As he says, it’s a “belts and suspenders” approach. We talk about everything related to the bridge unroofing procedure. The fear patients have around sternotomy,The diagnostic process including provocative testing,The reality of competitive flow in grafts,The insurance coding challenges that can delay care,And, perhaps most importantly, how critical it is to advocate for yourself.Whether you're a patient, a caregiver, or a provider trying to better understand this often-dismissed condition, I believe this episode can give you clarity, confidence, and hope.In This Episode, We Cover:Dr. Kwon’s path from aspiring engineer to heart surgeonWhy sternotomy isn’t as scary as it soundsHis unique method: unroofing + bypass for added protectionWhen provocative testing is useful—but not always requiredWhat to know about LIMA grafts and long-term outcomesWhy some cardiologists still dismiss this condition—and what he tells themThe importance of second opinions and listening to your gutHow insurance coding can make or break your surgery approvalPlus... This is an encore episode as I believe we cannot stress the significance enough for this type of testing to confirm the effects of a myocardial bridge on the patient. The more that's known, the more likely it is that the proper course of treatment can be taken. If you missed it the first time, don't miss it this time. This is a must listen episode.Unlock the secrets of properly understanding and diagnosing myocardial bridges with insight from Dr. Jeffrey Fowler, a leading interventional cardiologist at University of Pittsburgh Medical Center. Our conversation promises to illuminate the often perplexing process of diagnosing these cardiac anomalies and their impact on heart function. Through the lens of minimally invasive procedures and advanced imaging techniques like coronary CT angiograms, Dr. Fowler demystifies why myocardial bridges are not always the main culprits behind chest pain and the definitive testing now being done to ascertain the significance of the myocardial bridge. We're going to explore the intricate world of invasive testing, as we detail the provocative testing process used to evaluate myocardial bridges, their severity and their contribution to angina, or chest pain. Dr. Fowler explains how starting with an empiric trial of medications can lead to more invasive procedures like heart catheterization if necessary. Learn about the steps of the provocative test and the role of acetylcholine in testing for endothelial dysfunction and vasospasm, with reassurances about the safety and reversibility of these tests. This episode is our first on the detail and best practice for diagnosis of the bridge through the provocative test we've heard so much about. It's meant for the education and knowledge for both patients and professionals, enhancing understanding of the nuanced approaches in relatively rare process of diagnosis.Celebrate the spirit of collaboration among cardiology experts as Dr. Fowler shares the necessity of communication and advice. He's consulted with many others along the way including Dr. Shaw at Yale and Dr. Tremmell at Stanford, highlighting real-world case studies like that of patient David Vollmer. This episode needs little description. Thanks to Kelly and Tank Sears for their candid and sometimes raw emotion in this conversation as they share the reality of the strain a myocardial bridge can put on a relationship, a marriage, a family. Yes, we'll cover the health portion of the surgery and Kelly's amazing recovery after a less than vacation like visit to Greece to see Dr. Kofidis, but it's the perseverance and trust that both she and her husband demonstrated in getting not only the appropriate health care but how they held it together as spouses in a truly challenging time. And that's not to mention the kids in the mix too. The strength of their love was challenged and the good news... there's a happy ending. This intro requires few words. We have a friend with a huge podcast and radio audience. 170 stations strong and he gave us the courtesy of interviewing me to help us get our stories told and raise the awareness for the MB condition. Dr. Ronald Hoffman, the longest running physician hosted radio show in syndication on 170 radio stations had no idea what a Myocardial Bridge was. He wanted to know and he wanted his audience to know once he took a look at the book. He gets it and he understood it well enough that he wanted to help us share it to his audience. He saw the value to both his listeners as well as the doctors that listen to his Intelligent Medicine radio show and podcast. With the deepest gratitude, I cannot thank him enough. We're making noise, we're getting traction and we're saving lives in our collective effort to continue to keep getting the word out. Thank you Dr. Hoffman for your interest to host me on "Intelligent Medicine Podcast"!The interview on his podcast is in this link: https://drhoffman.com/rethinking-heart-health-uncovering-myocardial-bridges/You can find out more about Dr. Ronald Hoffman by visiting his website https://drhoffman.com/

I hope you can use it to your advantage to get the story told that Myocardial Bridges are real and they do cause symptoms. Let’s use the book as a tool to educate, inform and provide hope. Thanks! This is an encore episode released previously in February of last year. Due to its popularity and the incredible journey of my guest, Kelly Portillo, not to mention the value of faith in many of the conversations had with patients, it seemed appropriate to post again. Certainty removes any semblance of doubt and without doubt, there is no reason for faith. Our condition is anything but certain, we and everyone supporting us are filled with doubt and fear and that means there’s a great big opening for faith. Discover how Kelly Portillo practiced her faith successfully. Discover the power of perseverance and the importance of advocating for your own health as we share Kelly’s captivating story, an individual who transformed her struggle with undiagnosed heart issues into a journey of self-discovery and medical triumph. At the young age of 13, Kelly’s life took a dramatic turn in a karate class when she first experienced symptoms that would lead her on a path filled with challenges, misdiagnoses, and a relentless pursuit for answers. Her experiences shed light on the complexities of navigating the healthcare system and the courage it takes to stand up for oneself when faced with skepticism from professionals who should be allies. Feel the frustration and eventual elation as Kelly, undeterred by failed multiple ablasions, discovers and then connects with the expertise at Stanford to tackle her condition head-on. Her story is not just about the physical ailment of supraventricular tachycardia and a myocardial bridge but also about the emotional battle with it all, including the insurance companies, the exhausting appeals, and the sweet success of validation. Kelly’s journey highlights the critical role of patient advocacy and the difference it can make in receiving proper care, offering hope and guidance for those who may find themselves in similar, overwhelming situations. Her constant and consistent belief in her faith and her fortitude, would yield incredible transformation from living with decades of pain to now embracing an active lifestyle post-unroofing surgery. Her narrative not only provides solace but also inspires; there is hope if you’re suffering from a myocardial bridge and with the right support and determination, a quality life after years of furstration, is more than just possible—it’s within reach. Enduring the tumultuous journey of heart condition diagnosis and treatment led me, Jeff Holden, to realize the profound need for a supportive community and reliable information. Reflecting on my own experiences, from the initial confusion and misdiagnosis to the breakthrough testing and subsequent surgery at Stanford, I’ve been driven to share insights that can empower others in similar situations. This episode marks a significant milestone with the release of my new book, Imperfect Heart: Stories of Myocardial Bridges, a tangible resource aimed at providing solace and understanding to those who prefer something beyond digital content. Through patient stories and expert interviews, I’ve assembled a wealth of knowledge that addresses the myriad challenges faced by patients, families, and healthcare professionals alike. Together with the relentless dedication of our Facebook group participants, the administrators, Rob and Stephanie, and the rest of the team, our community has flourished, expanding from 1,200 to over 2,600 members in just two years. This growth underscores our collective mission to raise awareness and foster understanding of heart conditions. It also emphasizes the need for a book as well. My hope is that this book will serve as an invaluable tool not only for individuals grappling with their diagnosis but also for medical professionals who need a deeper grasp of patient experiences. By sharing these narratives and insights, I aim to reach those who aren’t digitally inclined and provide a cornerstone of support that bridges gaps in communication and knowledge across all fronts. Now we finally have… a book, the book, my book. Get your copy of Imperfect Heart:Stories of Myocardial Bridges HERE.
Jennifer Shetler never imagined that a routine run would lead her from the role of a healthcare professional to that of a heart patient. Join us as Jennifer shares her deeply personal journey through the complexities of cardiac care, from initial missteps in diagnosis to the eventual discovery of a myocardial bridge. Her story emphasizes the unpredictable nature of heart conditions, particularly for athletes, and sheds light on the unique ways these issues present in women. As we navigate Jen’s experiences, we highlight the critical steps in diagnosing and managing cardiac conditions, emphasizing the importance of comprehensive testing. Her narrative underscores the challenges faced when outward appearances of health mask underlying problems, and how persistence in seeking advanced imaging can be life-saving. She candidly shares her decision-making process surrounding surgical options, the role of community support, and her reliance on a network of friends and run club members in overcoming her health crisis. The episode also explores the emotional and physical journey of recovery following cardiac surgery. Jennifer offers insights into building a strong support system and the power of patient advocacy in securing necessary care. Through her story, listeners are reminded of the importance of listening to their bodies and advocating for themselves within the healthcare system. Jennifer’s transformation from heart nurse to heart patient serves as a poignant reminder of the value of resilience, knowledge, and community in navigating life’s unexpected trials. Her trust in care is clearly demonstrated knowing the track record of a cardio thoracic surgeon known for his skill in bypasses, not unroofing procedures. This should be a valuable understanding as it opens the door for so many more opportunities to be “unroofed” knowing the necessary content  is available to teach and support a qualified surgeon to perform the sugery he or she has likely already experienced.

I was doing some work with AI for a project and I wondered what I would find if I began a conversation about myocardial bridges. To my amazement, the responses to my questions were quite contemporary, on point for the most part and very much aware of the condition without doubt. I found the answers to be as technical as I wanted to get or as general given what I asked. The overall takeaway for me, was that there was little downside or disinformation. The entire conversation was pro myocardial bridges as we know them. If we could only get the majority of our medical community to be as receptive to the condition as our AI is, there would be far fewer people suffering from the condition and, in my opinion, far fewer deaths from cardiac arrest. We’re making progress and the more our digital/virtual resources endorse the corrective procedures for a condition that is more common than other diseases like cancer and stroke, the better off we’re going to be. There’s a lot in the episode as we cover a lot of ground. It’s one worth taking notes on if you find areas of interest. I hope the episode gives you some additional support to share with whomever you’re trying to convince that what you’re experiencing is real. It should also enlighten you to the capability of what AI can provide us with regard to information about myocardial bridges. It gives me tremendous encouragement to see that AI is initially very receptive to the techniques for correction of a bridge.  Here’s to a future of acute awareness of MB’s and to procedures and techniques to remedy them. I know one place we can go to get comfort in knowledge! Enjoy the episode.
In this episode I take a moment to reflect and celebrate a year of content novelty and camaraderie in our Imperfect Heart community. From hosting 11 awesome patients who shared their personal journeys to featuring insights from 10 esteemed doctors, this episode is a testament to the strength and resilience of our community. We explored the world of unroofing surgery with top robotic surgeons and experience the detailed moments from inside the OR through exclusive videos on our YouTube channel. Each story and professional insight is a stepping stone toward understanding and advocacy for myocardial bridges and for that, I’m grateful. Our first-ever Myocardial Bridge meetup united patients from across the nation, sparking a network of support and hope. We even broke new ground with a three-part series chronicling a patient’s journey from diagnosis to surgery, offering a rare, in-depth look at the procedure. An interventional cardiologist joined us to clarify the provocative test, an essential but often misunderstood evaluation for our condition. What a wonderful year it’s been. I invite anyone with their own story or a recommendation for a medical professional to join our mission of raising awareness and support. Together, let’s continue to shine a light on myocardial bridges and inspire each other.  Have a wonderful Christmas holiday and here’s to a happy and healthy new year in 2025.
