Jennifer sent in a great question to "Ask the Egg Whisperer." She is 38, and from Seattle. Here's what she wrote," Hi Fertility Queen, you are the BEST! I'm 38 years old & diagnosed with diminished ovarian reserve. I've been through 5 rounds of IVF and have 2 5AA frozen embryos right now. My husband and I want 2 kids therefore tried to bank another embryo but the 5th round failed therefore we are moving on to a FET. I did a mock transfer and the BCL6 from the Receptiva came back positive therefore we are doing 2 Depot Lupron shots a month apart prior to the real transfer. My environment is good (healthy eating, lots of exercise, limited toxins in our beauty and cleaning products, great therapist). I'm taking a Prenatal with folate and DHA, Vitamin D, & Vitamin E. Are there any other supplements I should take? I've heard NAC may be good for endometriosis. Is there anything else I can do to prepare for this transfer? I am willing to do whatever it takes to help my pregnancy chances. Thank you so much!" The brief answer for Jennifer is this: 1. Have you done the ERA test on the two embryos you have? 2. I'm delighted to have Dr. Ramy Ghayda join me today to talk about sperm, and the importance of sperm DNA Fragmentation testing and how to improve sperm health. When trying to conceive it takes both an egg and sperm cell. I'm so delighted to have Meaghan Doyle back on the show today to talk about the truth about abnormal embryos. Meaghan is a genetic counselor, and founder of DNAide, where she specializes in infertility, preimplantation genetic testing, and preconception genetic counseling. In this discussion, Meaghan and I are talking about the different kinds of embryo testing, what genetic test results should be used for, and why all fertility patients need to ask for reports on "abnormal" embryos. The truth is that sometimes, patients are told that their embryos are abnormal but never learn much more about the embryo itself. In some situations, those abnormal embryos can become healthy pregnancies and healthy, live babies, so it's important to learn as much as you can about all of the results of the genetic testing done on your embryos. To set the stage for this conversation, I asked Meaghan what it means to genetically test an embryo, and here's what she explained: "It means that we've taken a sample from the outer layer of the embryo, usually a few cells, and sent that sample to a genetic testing lab. And there's actually a few different types of genetic testing that can be done on that sample. Generally, when we say an embryo has had genetic testing, we're referring to a test called PGT-A. And so when we say an embryo has been genetically tested, usually that's what we're referring to." So what happens when an embryo comes back as "abnormal," and what can you do if it happens to you? Today I'm joined by Dr. Anna Glezer, MD and we are talking all about how to stay in top mental health as a fertility patient. Fertility patients know that treatment feels like being on a roller coaster. It's a time that can be full of optimism and disappointment, and because it can feel isolating, I include "Therapist" in my foundations of building your own Fertility TEAM (Therapist, Eating healthy and Exercise, Acupuncture, Mindfulness and Meditation are the basics). I love that Dr. Glezer explains how to approach the whole picture of mental health: getting support, taking good care of yourself, getting enough sleep, finding someone to talk to, eating well, doing some physical activity, and finding an integrative psychologist to work with if that feels right. As you'll hear her say, "For anyone who's listening, who might be having a little bit of a hard time with their emotional and psychological health: you're not alone. This is really common. Dr. Geoffrey Sher is back, and we are talking about Intrauterine Insemination, or IUI. In 1982, Dr. Sher and his colleagues were the first to wash sperm, and then put it through a centrifuge process. After an incubation period, they used a catheter to inject the sperm into the uterine cavity of a patient. He's a true pioneer, and this was the first time IUI had been done!. I know you will enjoy learning more about IUI from him. We're sharing about how IUI works, and why it's recommended for some patients. I'm so excited to have Dr. Bruce Lessey of ReceptivaDX join me on the podcast. Dr. Lessey has dedicated his research life to studying the lining of the uterus and more specifically, how to help embryos stick better. His research led to the discovery of the role of BCL-6 in the lining of the uterus as a diagnostic test patients can have done by their doctors to guide them toward the best protocol for transfer success. The test isn't for women just going through IVF either. This test can also be done to help women get an endometriosis diagnosis without going through a laparoscopy surgery. ⠀ ⠀ In addition to being a Fertility Doctor at The Fertility Center of the Carolinas, in Greenville SC, Dr. Lessey is also the scientific advisor for Cicero DX. Is your pregnancy test negative, again? Let's get real. It sucks when we do everything we can in our power to make something happen, and it just doesn't. If you've ever wanted to get pregnant, and have seen a negative result on the test, you know exactly what I'm talking about. It doesn't matter if it's one month of trying or 2 years…. "Trying to get pregnant" can be painful, and the phrase itself is defeatist. The mindset it establishes is one based on fear vs. love. I like to lead from a place of love, and encourage my patients to think positively. For that reason, I advocate for replacing it with, "preparing for pregnancy." Doesn't it just sound more peaceful, and open to the process? I think so. Regardless if you are on board with my line of thinking, I do want to share some helpful tips and suggestions so that your path to pregnancy is as informed and efficient as possible. Here are my top tips: My #1 tip for preparing for pregnancy is to NOT put blind faith in fertility apps. Tip #2: Develop a resilient, and positive mindset. Tip #3: Make use of science (diagnostic testing) to understand what could be preventing you from getting pregnant. Oh, and ask questions. Lots of them! I'm thrilled to have joined Dr. Lorne Brown on the Conscious Fertility Podcast to dive deep into the world of platelet-rich plasma (PRP) therapy and its potential for women on their fertility journey. It's a delight to have Dr. Geoffrey Sher back. He's been on the show several times, and our discussion on "How to Avoid Implantation Failure During IVF" is one of the most viewed on the channel! Today we're talking about treating Endometriosis - related infertility. Here are some facts about Endometriosis as shared by Dr. Sher: - Endometriosis is a condition where cells similar to the lining of the uterus (endometrium) grow outside of the uterus, in the pelvic cavity. - It occurs in about 10% of all women and can be symptom free or may cause a wide spectrum of symptoms. - It's estimated that 25% - 40% of all female infertility is linked to endometriosis in some way. - Endometriosis is a life-long condition. It cannot be completely removed or cured. While endometriosis can usually be controlled (medically or surgically), the condition itself is not curable. - Removing endometriomas from an ovary may help with getting pregnant. Dr. Sher is also sharing information about how the endometrium begins to grow outside the uterus, and why some women remain asymptomatic, and others develop symptoms.
