The single most expensive mistake in your dental career might not be a failed marketing campaign or a bad hire—it's likely hidden in the pages of the lease agreement you signed years ago. While most practice owners focus on the monthly rent, the real financial traps are buried in complex clauses that can dictate the future value and salability of your entire practice. In this critical episode of Dental Unscripted, hosts Michael Dinsio and Paula Quinn sit down with dental real estate specialist Dan Gleissner of Ally Healthcare Real Estate to expose the pitfalls that most dentists and even non-specialized agents miss. Before you sign or renew, you need to understand the long-term consequences of your lease. In this discussion, you will learn:The "Sale-Killer" Clause: About half of all first-draft leases contain severe "assignment" language that could prevent you from selling your practice. In rare cases, just requesting to sell can give the landlord the right to terminate your lease immediately. The Truth About Triple Nets (NNN): Your rent is not your total cost. We break down the three "nets"—property taxes, insurance, and Common Area Maintenance (CAMs)—and explain how these estimated expenses can lead to a surprise bill for thousands of dollars at the end of the year. The HVAC & Roof "Gotcha": Who pays for a $30,000 HVAC replacement? In many retail leases, the tenant is responsible for the full, immediate cost. Learn how to negotiate spreading these capital improvements over the lifespan of the asset.eThe Two-Year Rule: To maintain leverage with your landlord, you must start the renewal process at least two years before your lease expires. Waiting until the last minute puts you on the defensive.00:49 Topic Introduction: Dental Leases 01:20 Guest Introduction: Dan Gleissner 03:42 The Risk of Non-Specialized Agents 11:00 Exclusivity Clause Explained 18:04 The Importance of the Assignment Clause 26:18 What is a Triple Net (NNN) Lease? 34:56 "Gotchas": HVAC and Roof Replacements 40:44 The Lease Negotiation Process & TimelineVISIT NEXT LEVEL CONSULTANTS:Michael Dinsio is a longtime dental startup consultantVISIT https://nxlevelconsultants.com FOR MORE INFO on how to partner with him on your dental practice startup! VISIT Dental Unscripted's WEBSITE:https://www.dentalunscripted.com HERE you can SEE all the past and current EPISODES. Why does patient financing matter? The team uncovers eye-opening stats: 15-20% of patients walk in unprepared to pay. A practice producing $1.6 million often sit on $2-2.4 million in unscheduled treatment. Michael Clark, a former CareCredit insider, explains how offering payment options bridges the gap, especially for bigger treatment plans like crowns, implants, or clear aligners—where "big" varies from a $1,000 crown to a $25,000 All-on-4 case. Paula shares her own experience as a practice owner, admitting she once cherry-picked when to offer financing, while Co-Host Mike Dinsio highlights a case where a $1.6 million dollar practice only used CareCredit once in an entire year. The result? A massive missed opportunity. The trio tackles why practices shy away—staff discomfort with money talks, misconceptions about merchant fees (not interest!), and the pitfalls of in-house payment plans that rarely get collected after 120 days. They cite Kathy Jamison’s research, revealing in-house plans cost 42 cents on the dollar to manage. Meanwhile, patients finance everything from mattresses to nose jobs—why not dentistry? With 50% of revenue from insurance, 35% from traditional payments, and a growing need for affordable options (average savings under $2,000), financing is a no-brainer 🧠We discuss options beyond CareCredit as a household name— new options to consider include Covered Care, Clarity Pay, niche players for larger cases like clear aligners. The key? Offer multiple choices (cash discounts, credit cards, third-party financing) on 100% of treatment plans, letting patients decide. Why? Paula notes patients are 50% less likely to schedule once they leave, and Michael Clark adds that 25-30% get treatment elsewhere if they don’t commit day-of. Credentials and reimbursements can vary per location so it can be insightful and strategic to mirror what the seller's existing insurance arrangements for a smoother transition.6:00 How to Prepare to Get Credentialed Getting accurate credentialing information during a practice transition can be challenging since brokers don't handle these details. Moreover, sellers are often unfamiliar with their own status. Front office team members manage this information and sellers may not request this information for fear of revealing a pending sale?! To verify individual doctor's credentialing status, dentists should proactively maintain copies of their insurance contracts. Direct contact with insurance networks to verify, and, when possible, inquire with front office staff about existing credentialing contracts. 14:50 Common Roadblocks and ChallengesCommon delays in the credentialing process for dentists include expired malpractice insurance, reliance on employer-provided coverage. Insurance companies often require a finalized bill of sale before approving contracts, further delaying the process. Keeping dental licenses and CAQH profiles updated is crucial, especially when transitioning between states. Working with professionals early on helps streamline the process, ensuring all necessary documents are in place to avoid unnecessary delays.19:10 How Long Does it Take to Get CredentialedWhile some claim it can be completed in as little as two weeks, the reality depends on factors like whether the provider is joining an existing practice or establishing a new entity with its own tax ID and NPI. The process involves multiple contracts, including terminating old agreements, which can cause delays. A best-case scenario suggests a 12-week turnaround, but errors in applications or insurance company delays can extend the process. Proper expectations and persistence are key to navigating credentialing successfully.28:20 What Does Being Out of Network Mean?! Failing to properly prepare for network credentialing can create significant issues for dental practices, especially when patients realize they are unexpectedly out of network. This can lead to frustration, distrust, and potential financial concerns. But many offices make the transition harder than necessary by not fully understanding, in the interim, they can still honor network fees while awaiting credentialing. Proper planning and clear patient communication are key to ensuring a smooth transition and avoiding unnecessary complications. The importance of working with a trusted, experienced professional over the anecdotal advice given by peers who probably lack the firsthand experience involved in the credentialing processes. 36:05 Working With Credentialing CompaniesThe extensive effort and tracking involved in credentialing, underscoring that their goal is to complete the process as quickly and smoothly as possible. Despite misconceptions, delays often stem from insurance companies rather than credentialing firms, and the representatives frontline callers interact with may not have access to accurate information. Ultimately, they urge more understanding and patience, reinforcing that passion and expertise drive Next Level Consultants to achieve the best outcomes for their clients.
