In this video we will examine loneliness, its impact, and how to manage it. I have create a worksheet to help you not only identify issues of loneliness, but also help you identify whom you can connect to to challenge those loneliness feelings. Loneliness is on the rise, but we are connected to each other more and more today than ever before. Loneliness is a lack of affection in social relationships, this includes in person or online. Loneliness is not actually due to being alone, but being without a sense of being needed or wanted in relationships, a loss of feeling connected. Loneliness has been found to lead to various psychiatric disorders like depression, alcohol abuse, child abuse, sleep problems, personality disorders and even Alzheimer’s disease. Individuals with BPD are more likely to experience periods of loneliness and many individuals with BPD have an intolerance for being alone and may reach a level of desperation where they will do almost anything to get rid of it. This can be getting into unhealthy relationships, using drugs and alcohol, or foregoing personal values to have someone, anyone, in your life, for example. You can counter and challenge those feelings of loneliness. It may very well be a core content issue, but that doesn’t mean you have to live with it. In the worksheet I’ve created for this video, see comments section for link, you will identify instances that remind you of when you felt lonely and alone within various age ranges, 5-10, 11-15, 16-17. I chose adolescence because many foundational experiences, such as loneliness, are first cemented at these ages. You can add information in any box for any age you like. In the final box, list what makes you feel connected to others now and with whom. This is really important. For example, do you feel connected to family or friends, what makes you feel connected, perhaps they ask how you’re doing, how your day was, they want to spend time with you, etc. Some people don’t have anyone they feel connected to, and in these cases I want you to try and establish social connections to others. This can be through meet-ups or other resources that get you out there. Staying put, staying on your phone doesn’t help. Social networks give you a shot of dopamine but not the same physical and neurological click we all need with physical presence and connection. Daniel J. Relationships are central to all of us, but particularly to those along the BPD spectrum. This video is going to discuss the favorite person relationship and some of the inherent challenges and fears as well as provide some suggestions to help you manage the relationship with your favorite person. A favorite person is one or two people that you absolutely idolize and want to spend all your time with, this could be a family member, significant other, friend, therapist, etc. Many are sensitive about their favorite person. If your favorite person is busy and can’t spend time with you, you tend to get angry and feel abandoned. You’re on pins and needles to have access to them how, when, and to what degree you feel you need at that moment. Many people experience extreme jealousy, separation anxiety, approach and avoidance behaviors, intensive tracking and monitoring of their favorite person, and they may be overly apologetic to try and maintain the relationship. These behaviors often do the opposite of what you want, which is to keep the favorite person close but instead your favorite person feels pressured and may need or demand distance. How can you manage this relationship? Build your sense of self. Learn about what makes you – you. For example, kick everyone out of your brain and you answer these questions as you see it, not for anyone else – what’s your favorite food, favorite color, what helps you relax, what helps you feel good, what makes you proud of yourself? These are questions to help you start learning about yourself. If you don’t know the answer, take time and learn the answers. The food one is the best. I hate onions but love chocolate for example. Challenge your negative thoughts and assumptions of what that person is. In your mind you may see them as the end all, be all of all people, but he/she is just a person, like you. What can you do for yourself that you feel you must get from him/her? Your negative thoughts and assumptions tell you that you cannot do it alone, that’s false, challenge those distorted beliefs, and try doing more for you without the need for your favorite person. It may be scary at first, but it can be liberating in the end. Cost and benefit relationship analysis. Do an objective cost and benefit analysis of the relationship. What does he/she bring to the relationship? What do you bring to the relationship? What’s healthy, what’s unhealthy? What you get out of the relationship? These answers must be clear and concrete, they can’t be something like “he/she loves me like no one else”. Define this, what does this mean and what does it look like? Remember your value and that you matter in all relationships and what you want and need has value. I wish you well. Daniel J. I think this is important for people to know so they can build knowledge into themselves or their loved ones. There are 9 criteria and you must have 5 or more to qualify for the diagnosis. All 5 of these must be moderate, severe, or extreme in severity. If you only have 4 or less, that’s traits and not the disorder according to the criteria. Remember, this video is not for you to diagnose yourself. You must go to a mental health professional for that. Understanding symptoms is one thing, but a diagnosis is something else, which can help you seek treatment so you can manage and control these symptoms that are causing you problems. You may get confused as I go through this, and that’s normal, because diagnosis is not cut and dry or an is or isn’t kinda thing. There are many factors that go into a diagnosis and PDs are particularly complicated. Criteria # 1. How great are your fears of abandonment and do you engage in desperate efforts to avoid this abandonment? Is it mild, moderate, severe or extreme? 2. Do tend to see others as heros or zeros, and does this alternating view cause disruption in your relationships, if so how much - mild, moderate, severe or extreme? 3. How much would you say you don’t know who you are, what you believe in, or what you stand for - mild, moderate, severe or extreme? 4. How severely do you engage in self destructive behavior, like spending sprees, sex, substance cause, binge eating, etc. - mild, moderate, severe or extreme? 5. Do you have reoccurring suicidal gestures, threats, behaviors or self-mutilation? If so, how often and how impactful are they in your life - mild, moderate, severe or extreme? 6. Intense emotional reactions due to instances in your life, such as intense depressive Sx, irritability, and/or anxiety that lasts a few hours only, rarely more than a few days. Is this mild, moderate, severe or extreme? 7. Feeling empty inside, like a hole you can’t fill. Is this feeling mild, moderate, severe or extreme? 8. Intensity of anger is out of proportion to the issue or incident that set you off. Is your anger mild, moderate, severe or extreme and does it occur often? 9. You feel like people are watching you, holding back, preventing you from succeeding, and only want the worst for you? Is this mild, moderate, severe or extreme? Daniel J. Suicide and non-suicidal self-injury (NSSI) is a very serious and regretfully highly relevant topic and issue with those with BPD and with BPD traits. The comments section for this video, and all others, are not monitored suicide prevention resources. Call the hotline or go to your local emergency room if you’re thinking of harming yourself. There is hope and help. Suicide Lifelines work, data shows that people who called were significantly more likely to feel less depressed, less suicidal, less overwhelmed, and more hopeful by the end of the calls. If you’re thinking it, call! Citations and resources are at the bottom of the comments section if you’re interested in learning more and want to access more information. Critical protective factors include: • Strong connections to family and community support • Skills in problem solving, conflict resolution, and non-violent handling of disputes; • Personal, social, cultural and religious beliefs that discourage suicide and support self-preservation; • Restricted access to means of suicide; • Seeking help and easy access to quality care for mental and physical illnesses. There is effective treatment out there for those with acute and chronic suicidal ideation and past attempts, particularly DBT. Have hope, you can get help. Finally, many think of suicide as a singular act, but it is not. It has a ripple effect on family, friends, acquaintances, and society as a whole. Did you know that reports of celebrity suicide are associated with increases in suicide rates and highly publicized suicides also drives suicidal behavior, attempts, and ideation. Completed suicide tells those you know and love, and even those you don’t, that this is a viable option, this is how you deal with stress and problems, but we know it’s not true. This error is even published in the DSM-5, the mental health diagnostic “bible”. More recent studies show no gender differences when controlled for these errors. This video is directed at clearing up this confusion to help you understand and recognize these differences. The gender problem persists as we believe that BPD presents in the same manner for both genders. By doing this, we miss key distinctive factors to accurately identify BPD in males. A lot of the confusion resides in females being diagnosed with BPD who actually meet criteria for ASPD, and males who meet criteria for BPD being misdiagnosed with ASPD. Men with BPD were more likely to have comorbid narcissistic, antisocial, paranoid, and schizotypal personality disorders, alcohol and substance use disorders but less likely to have dependent and obsessive–compulsive personality disorders compared to women with BPD. For those who may not know, Schizotypal PD is characterized by discomfort in close relationships, cognitive or perceptual distortions, and eccentricities of behavior. Daniel J. What is an obsession: an idea or thought that continually preoccupies or intrudes on a person's mind? This increases anxiety and drive for impulsive behavior to lessen the lessen the anxiety and discomfort. Obsessions covers a lot, so I thought I will discuss this in the context of OCPD and its connection to BPD and treatment options. You might be surprised what you learn, let’s check it out. • OCPD is characterized by excessive perfectionism, rigidity to rules and morals, orderliness, and control to the level that it impacts your functioning. It is considered one of the most prevalent PDs between 2.1% and 7.9% of the general population; BPD is estimated between 1.6% and 5.9% in the general population. • There is some research evidence to suggest the presence of some early experiences leading to emotional difficulties in those with OCPD similar to those seen in other personality disorders, such as child abuse, anger, suicidality, and substance use; in one study, 72% of participants diagnosed with OCPD reported a history of childhood abuse, including 36% endorsing sexual abuse and 81% endorsing neglect. Participants with BPD tend to expereince stronger and more negative emotions (anger, hate, resentment) and greater problems with emotional control that drive behavior than those with OCPD. Individuals with OCPD endorse strong negative affect (including anger) that those without OCPD, and they have difficulty accepting, understanding, and managing their negative affect effectively. However, they report being able to control the expression of these emotions whereas those with BPD, report being unable to keep impulses in check. Degrees of difference. They both tend to have difficulty completing tasks and concentrating when upset, but this may be due to the tendency of both disorders to having problems with rumination, fixations, or obsessions with thoughts and preoccupational thinking (“I can’t get it out of my head”). This is a complicated combination that poses many challenges to not only the individuals who have these diagnoses but to the mental health practitioners who treat them. Attention-deficit/hyperactivity-disorder (ADHD) is a neurodevelopment disorder characterized by inattention, hyperactivity and impulsivity. BPD is characterized by erratic and dramatic responses to internal and external stimuli. ADHD occurs in in about 5% of children and about 2.5% of adults and is frequently comorbid with mood, behavioral, substance, and personality disorders. Research has found that: 60% of those Dx with BPD as adults were Dx with ADHD in childhood. 18 to 27% of adults with ADHD have comorbid BPD. Individuals with ADHD and BPD have more problems in school or work, greater substance use, and more negative interactions with police. There is a lot of confusion about the intersection and co-occurrence of BPD and ADHD. In adolescents and adults, it is difficult to distinguish ADHD because both disorders tend to share the surface structure features of disorganization, impulsivity, social intrusiveness, emotional dysregulation, and even cognitive dysregulation. Distinguishing characteristics between BPD from ADHD are those issues often seen in those with BPD includes: fear of abandonment, emptiness, worthlessness, or other core content features of BPD. Impulsivity is where the greatest overlap exists, it is consistently identified in both through the research. What happens when we base who were are on likes, hearts, views, and other social media and internet rewards? Research shows that we become more unhappy and disillusioned with who we are and the world around us, but we also become addicted. In this video were going to talk about Social media and internet addiction and how it relates to personality. Research findings supported the notion of addictive social media use related to a need to feed the ego (i.e., narcissistic personality traits) and it inhibits a negative self-concept (i.e., basic cognition) and increase a sense of positive mood state. Results also show that females are at a higher likelihood to develop more addictive use of activities involving social interaction than males. A strong predictor component of social media and internet addiction is Fear Of Missing Out (FOMO). This could also qualify as fear of missing that text or DM response. Y Narcissism was positively related to addictive use of social media. It is believed that Facebook, Instagram, Snapchat and other social media applications may serve as ideal social avenues for individuals who desire and are attracted to engaging in ego-enhancing activities. It has been speculated that individuals with elevated narcissistic traits use social media excessively because these online platforms may fulfill a need for affiliation and confirms the sense of an idealized self. This is in line with studies showing that narcissism is positively related to profile updates regarding accomplishments, diet, and exercise. What about folks with BPD traits? Research illustrates that those with BPD traits are at a higher likelihood to have unstable interpersonal relationships with others, negative emotions and unstable personality, those with depressive, anxiety and dissociative symptoms, which may be related with emotional abuse, may find it easier to interact online with others in which anonymity can be maintained rather than engage in contact in the real world. Additionally, this finding may be that individuals with negative emotions, such as anxiety or depression, may use dissociation and/or the Internet to relieve these emotions both internally and externally. This term is often misused colloquially referring to someone who commits a mean or very hateful act. When we talk about psychopathy in this video we are talking about people who have a long history of these acts and in many cases they’re extreme, but there are other factors to consider as well that we’ll discuss. We’re using the term clinically, not colloquially in this video. Psychopaths operate at such a level that they have a high likelihood to end up incarcerated, have poor employment history, are erratic and destructive in relationships, manipulative, aggressive and violent, lack remorse or guilt, have an impaired ability to learn following punishment, take advantage of others, are promiscuous, and so on. What about those who meet criteria for psychopathy but are able to, for lack of a better word, function in society and stay out of prison? These are what we will call successful psychopaths. What is Psychopathy and How Common Is It? Genuine psychopathy occurs in less than 1% of all non-institutionalized males age 18 and up. This equals approximately 1.1 million adult males in the United States. Out of the approximately 6.7 million adult males who are in prison, jail, parole, or on probation, about 16%, or 1 million are psychopaths. This means that approximately 93% of adult male psychopaths in the United States are in prison, jail, parole, or on probation. Compared to the unsuccessful psychopath, the successful psychopathic traits include higher level of autonomic responsivity, such as skin conductance, heart rate, respiration, blood pressure, as well as greater executive functioning, such as selecting and successfully monitoring behaviors that assist the achieving goals and also include attentional control, cognitive inhibition, inhibitory control, working memory, and cognitive flexibility. It has also been found that successful psychopaths have higher scores on conscientiousness – which is a personality trait where high scorers tend to be hardworking, dependable and organized, and higher levels of assertiveness and sensation-seeking but with intact cognitive empathy. Imagine you possessed a mixture of manipulativeness, aggressive and violent tendencies, lack remorse or guilt, no remorse about taking advantage of others, and promiscuous, but without the impairments that impair your ability to successfully judge the environment that would get you incarcerated for it. Successful psychopaths are in organizations and may be working beside you. They tend to get hired because they make a strong positive impressions at interviews, by seeming eager and driven, possess good ability for the job (thought they may be lying), appear emotionally well-adjusted, friendly and easy to get along with. Psychopaths’ undesirable personality traits may be easily misperceived as their irresponsibility may be misunderstood by employers as risk-taking or entrepreneurial spirit; sensation-seeking may be seen as high energy and eagerness for the job; superficial charm as charisma.
