Pam discusses the unfortunate default language, i.e. "complacency" and "failure to pay attention", often used post event. Use of these terms indicate a lack of understanding of human performance principles and does not offer value in event analysis.
Culpability is the gorilla in the room for most managers trying to understand Human Performance (or The New View of Safety). For decades organizations have managed safety under a model of "crime & punishment". This paradigm is so prevalent that many have a difficult time getting their minds around the fact that not everything is a "safety crime".
