When amateur boxer Jessica D'Abrosca entered perimenopause, she didn’t know what hit her. The exhaustion. The night sweats. The joint pain. She felt like she was unraveling. At 65, Diane Grim was the oldest woman competing at the Western States 100–widely considered one of the toughest organized trail runs in the United States–this year. It was her first ever 100 miler and she didn’t even start running till she was in her 50s, but as she says, every mile, every step is a gift, so why not? She did it. At 10:06 a.m. on May 24th, two-time guest Jeannette McGill stood on top of the world. Jeannette, 52, summited Mount Everest after three years of navigating hot flashes, heavy, erratic periods, joint pain, stubborn injuries, brain fog, and maybe most difficult of all for a high altitude mountaineer–newfound fear that made tasks that once came easily feel overwhelming. How many times have we heard that weight loss and maintenance is simply “calories in, calories out?” Well, the equation may be rooted in thermodynamics, but the reality is biology, history, hormones, and a whole lot more. This week, endocrinologist and Harvard obesity researcher Dr. Jody Dushay breaks down why this concept is far more complex than the numbers on a food label or how many calories our devices tell us we’ve burned. From how your body actually absorbs calories to how (or not) it expends them, we explore how and why weight gain and loss anything but straightforward—especially during menopause, when shifting body composition, gut microbiome changes, altered insulin responses, and evolving hormone levels, create a whole new metabolic landscape. As you’ll see, your body isn’t a calculator—it’s a dynamic, adapting system. We also revisit GLP-1 drugs, which are increasingly popular with midlife women, and where they fit in this whole equation (so to speak). Jody Dushay, MD, MMSc, is an endocrinologist at the Beth Israel Deaconess Medical Center and assistant professor of medicine at Harvard Medical School. She is also the founder and director of Well Powered, a comprehensive wellness and weight management program. Dr Dushay has studied the effect of pharmacotherapies and dietary interventions on body weight for years, including a large single-site study examining the effects of a glucagon-like peptide receptor agonist on body weight in obese women without diabetes. As a triathlete, she has qualified for Kona 3 times and came in 2nd in her age group at the 2020 Ironman World Championship in St George. Former track and field Olympic champion Marion Jones, who lost her record-breaking 5 medals and served prison time in a high profile doping scandal, thought that her hardest days were behind her. And in that realm, they certainly are. But now she’s facing a challenge she never saw coming: perimenopause. This week, Marion opens up about all of it–navigating her past, moving forward from her mistakes, trying triathlon as she approaches her 50th birthday, and the wrecking ball that is perimenopause. From learning to swim to navigating knee pain, hormone shifts, and public scrutiny, Marion shares what it means to start over—and live with gratitude—no matter where you are in life.Marion Jones became the first woman to win five medals in a single Olympics when she won three gold and two bronze in the 2000 Summer Games in Sydney, Australia. She appeared on the covers of Time Magazine, Vogue, and Sports Illustrated and became a household name. Though she never tested positive in a drug test, in 2007 Marion confessed that she had lied about taking performance enhancing drugs and was subsequently sentenced to six-months in prison and surrendered her medals. In 2010 she returned to her basketball roots, having led the North Carolina Tar Heel team to the 1994 NCAA Women’s Basketball Championship, and played for the WNBA’s Tulsa Shock for two seasons. Marion is now a speaker, entrepreneur, and an expert trainer and coach. Ingrid Harm-Ernandes, PT, WCS, BCB-PMD, is a board-certified women’s health physical therapist with over 40 years of experience, including nearly three decades specializing in pelvic health. During her tenure at Duke University Health System, she treated patients across urogynecology and physical therapy clinics, co-directed the Women’s Health Residency Program, and contributed to national pelvic floor research through the Pelvic Floor Disorders Network. A recognized leader in her field, Ingrid has served on committees for the APTA and AUGS, is a Medical Advisory Committee member for the National Menopause Foundation, and authored both scholarly articles and the book The Musculoskeletal Mystery: How to Solve Your Pelvic Floor Symptoms.
