Class II’s are only easy when they are small (but not too small), supragingival, easily accessible with straight-forward anatomy. In other words, about 1% of the Class IIs we encounter - because the vast majority I see are subgingival, wide, with awkward root concavities and tricky access. I went from using sectional matrices 97% of the time to now just using them 30% of the time - this is thanks to a circumferential matrix I started to use last year which is a game changer. I now do not need to use a wedge in most scenarios (something I used to think was a crime!) and have been using the ‘contact opening technique’. Meet Dr Sunny Sadana, who has brought the Greater Curve system to the UK and taught me a lot about efficiency with direct restorations. It also helps suppress the papilla to get better access to the caries. This can all be achieved by numbing the area first and wedging with adequate enough pressure for there to be separation. Like, going back to basics, you put a wedge in for your class II, and it ensures that the matrix is right up against that cavity margin. So, wedging is important. Jaz’s Introduction:The other benefit, of course, is that it gives you some separation. And this helps to accommodate for the actual width of the matrix span, so that once you finish your restoration, you remove the wedge and hopefully the teeth will rebound and you'll have a contact. Another benefit of wedges is actually suppressing or depressing that papilla. It allows you to manage those deeper cavities with much more ease. So as you can see, I'm a fan of wedging and I even invest in all sorts of different types of wedges and those low profile diamond wedges for those trickier and deeper scenarios. So why is this episode on doing your class two restorations without a wedge? Isn't that a little bit crazy? Hello Protruserati, I'm Jaz Gulati and welcome back to your favorite dental podcast. So let me tell you, it was like a paradigm shift. It was a big shift in my thinking when I started to use this technique without a wedge because the wedge has so many benefits, but how many times have you introduced the wedge and seen that your matrix band has completely now moved away from your contact? You have an open contact now. And sometimes a bit like the ring, the wedge can distort your matrix. So if there is a way to do good quality class three restorations and get a contact and get a seal without needing to use a wedge, then that is worth exploring. And that's exactly what we're doing today. My guest, Dr. Sunny Sadana is a major fan of the greater curve matrix band. He's a distributor in the UK. So because we're talking about tooth wear cases and the actual treatment of them, i. e. usually increasing the vertical dimension, aka opening the bite and how you fill in that space. Jaz’s Introduction:So if you are earlier in your dental and occlusion journey, or maybe you're not yet comfortable going beyond single tooth dentistry, then this episode may be difficult to grasp, but you should use these experiences as motivation and inspiration. It's a bit like when you go to a conference and some of the stuff that's being presented is so far beyond and above you at that moment in time in your journey where you are, but it's still really important to pay respect and listen because that can be the spark that you need in your career sometimes. But of course, if you're into occlusion and tooth wear. And you love restorative dentistry, then this could be a really geeky discussion that you're going to be able to apply on Monday morning if you've never used this technique before. Like, to give you a flavor of it, it's essentially using the wax up to create your own composite restoration. So you're using the wax up to create your own, like, direct composite onlays, for example, right? Direct composite onlays that you're making. So not indirect, it's direct because you're making them using the wax up and using the putty. And you'll see later, our guest, Zohaib Khwaja. He's going to explain why this has certain benefits and perhaps when he wouldn't use this technique and go indirect. And from speaking to him, I do think that it's a really nice trick to have up your sleeve, especially if you're interested in the adhesive management of tooth wear. Whilst I've got episodes on all different sorts of topics like we usually do on Protrusive, we're starting off with this orthodontics podcast. And let me tell you, if there's one episode you've listened to on Protrusive around the theme of orthodontics, make it that. Jaz's Introduction:This one, we cover such a great breadth of topics with my guest, Dr. Daniel Neves. We were actually recording. We actually sat on a sofa watching the sunset in Valencia, Spain, while we were recording this podcast. It's a special one is a great energy, great wisdom from our guests. So I'm really excited to share this with you. Look, the kind of themes that we cover is get my notebook out. The kind of themes you cover are anterior crossbites in children. This is actually what stemmed this podcast. Me and my colleague Suzy, we were feeling pretty bummed because we saw this kid, I think he was like eight or nine years old, and he had an anterior crossbite. And we thought, wow, this is really suitable. We think this would be a great case for an orthodontist within our publicly funded system in the UK to do some interceptive treatment for this anterior crossbite. And we were bummed because the orthodontist declined it. The orthodontist said, let's wait until age 12. Let's wait for all the teeth to come through and then refer this back. And I was really left scratching my head like, Hmm, hang on a minute. I'm pretty sure an anterior crossbite is an indication for early treatment. So let's find out what Dr. Daniel Neves had to say about that. We also at the end talk about root resorption. What if you have root resorption? How big of a problem actually is that for our patients? And then what if they experience some relapse and they want to have some aligners? Is it safe to do aligners and little minor movements on patients who have suffered with root resorption? Another one we covered. Again, all these questions are from you guys, the Protrusive Dental Community on our app, Protrusive Guidance. It's been so great to engage with a Protruserati. And so the question was bisphosphonates. What do you do with patients on bisphosphonates when you're considering orthodontics? What about doing aligners with someone who already has implants? What extra precautions or measures should we be looking at? What about fixed retention? Is it actually forever? Is there a time where we should perhaps consider removing that fixed retention or changing it up? We also discuss diastema.
We need to talk about the ‘S’ word and our mental health. I never, EVER want to lose a colleague again due to the pressures within Dentistry. And you're thinking, wait, hang on a minute, Jaz. We're in May. Why are you telling us about April? Well, I had a stressful month myself, actually, my youngest son was on and off sick and like nothing too serious, thankfully. Like it was antibiotics. Jaz's Introduction:It was tonsillitis. It was a couple of bugs. It was a night in A&E. And it really made me grateful in a way, in the sense that look, yes, my son was sick and I hated it and I felt really bad for him. And it can take your toll when your children are not sleeping very well. But it really made me feel bad for parents who had children with chronic illnesses, think, I don't know, leukemia, or parents with children who have severe learning disabilities, and having to care for those children, it really takes its toll on a parent. Or someone maybe with a child's got cleft lip and palate, feeding issues. Or of course Nafisa, the little girl in Tanzania that we're supporting, which Protrusive are supporting as a charity to help her raise money to fight her SMA type 1 so she can get genetic therapy. If you've been reading my emails, I put a donation link there and we're pretty much there to raising enough money to help this little girl. But anyway, going back on topic, it's been a stressful month, which is why this episode has spilled over into May. A few years ago, I probably would have been stressing about this, like, oh my god, it was supposed to be out in April and that would have eaten me alive. But my mindset has shifted a bit. It's a silly little story, actually, in terms of this mindset shift. Okay, the story goes that my son was on a playdate. It was like an early afternoon thing. And children nowadays, they have better social lives than their parents. So after this playdate, he had a birthday party to go to. So we were driving my son to the birthday party and we were really late. Like we spent too much time on the playdate and we were really late for the birthday party he had to go to. I hate, I absolutely hate running late. So, I was in a stressed state. If you ask me then, Jaz, how do you feel? I'm like, oh my god, I'm so stressed, we're late, Ishaan's gonna miss everything, the cake cutting, everything, he's gonna miss it, this is so stressful, and of course, two kids in the car, and that can get sometimes quite stressful. So, I reached the birthday party, and yes, he just about made it for the cake cutting,

I'm not exaggerating. It is Stress Awareness Month and what me and my guest, Jorge Andre Cardoso, what we cover is just amazing. Mostly him, his brilliance just exudes. And we cover such hard hitting themes that if there was one episode this year of Protrusive that you listen to or watch, make it this one. I have for many years, I still do, out of choice. But for some people, they don't want that. They don't want that to be a feature of them as a dentist. They don't want to have to spend their time treatment planning while their children are waiting. We also talk about the frustrations of there being so many different ways to do things in dentistry. Like you ask one colleague and they give you some advice and then when you ask the second colleague, that's when you've shot yourself in the foot because now you're getting conflicting advice. You are confused and that's stressful. We also cover themes like how much holiday should we be taking and how many days per week should an ideal dentist, how should a day look like, actually choreographing your day to minimize stress. And in my opinion, stress is a funny thing. Like, we know we need it.

And it's overwhelming when you're learning and you realize, oh my goodness, there's so much to check. You try and remember the oral medicine lectures. They tell you to check every single lymph node under the sun. Jaz's Introduction:And then of course, you remember the occlusion, the very few occlusion lectures you had, and you get the patient to bite together and you don't know what you're doing. You don't know what you're checking for, but you're just checking that the bite exists. And then to try and compute all the things you're seeing and try and put it in some sort of diagnosis and explanation, when you're a student, it's tricky. Which is why I've got Emma, our Protrusive Student on today to talk about routine, just routine, gold standard, daily examinations, something that you will see in general practice in the future. Whilst I am recording with Emma Hutchinson, the student, this is suitable for anyone who wants to learn, any dentist at any stage of career, who just wants to get back to basics, back to foundations. For a, what I call a Real World Examination. The reason I call it a real world examination is because the things that I'll be discussing today and what I'll be sharing video form on the Protrusive app under the student scholarship section is a real world exam. So, for example, a comprehensive exam evaluation could take an hour, 90 minutes. You ask different people. Some people do two hour full examinations. And they have my highest respect, and that's amazing. But that level of service is not for the masses. It's not for the general public. Look, some of my colleagues just get 10 minutes to do an entire examination. So I've had to find somewhat of a halfway house. When the patient feels pain during an ID block. When a matrix does not seal the cavity perfectly. When the shade of your crown doesn’t blend as well as you would like. When the endo you did last year failed. All of these cause us anxiety. All of these are failures in some form, and as Dentists, it eats at us. Let’s admit it: we are perfectionists. Life isn’t about being perfect, but it is all to do with the effort we apply to better ourselves day to day. Failure is a major part of success, but being hung up on our shortcomings will only lead to a downward spiral, which in turn will affect our results and general practice. I've got rubber dam on, the patient can't talk, the tongue is abolished, and I'm just falling in love with a little micro steps that makes our profession so much fun, right? Jaz’s Introduction:And when you grind your teeth left and right, the canines, for example, are taking a load at that point. And when this is happening the patient's joints are in centric relation. So the condyles are as fully seated position is snug position, and there are no quote unquote interferences or slides. I don't worry if you don't know what those things mean just yet, but some of you may remember seeing this. Maybe you don't understand or internalize what this actually means, but you learn what the perfect occlusion was. But then you started to see patients and you realize very quickly that these naturally dentate patients, your normal patients with the normal teeth. Whether they were 30, 50, or 70, they had all sorts of restorations, or sometimes no restorations. But they had anterior open bites, cross bites, and none of it looked like the bite in the textbook.

So it wasn't subtle changes. It was this lady has had treatment before and her history was that she had been to a number of other practitioners and she wasn't happy. So massive red flag right there. [Katherine]Okay. So she's seen lots of people, she's had lots of treatment and she's still not happy. Okay. And the second one, like I said to you earlier, is I often give my patients a mirror and then get them to kind of point out what their concerns are. Sorry, had to get that out of my chest, but we're back. We're back from AES TMJ conference in Chicago. I also went to Midwinter, and this episode will summarize mine and Mahmoud's trip to somewhere where we had longed to go for so long, the American Equilibration Society. Now, This society likes to be called AES because we've kind of moved away from the whole equilibration thing. Jaz's Introduction:It's way more than equilibration. It is occlusion. It is TMJ is the definitive conference sense that brings together all these occlusal camps. And I tell you guys, I was in heaven. I was in education heaven, and I was also in food coma heaven. So join me in Mahmoud on this, like on the field kind of different podcast, gain a sense of the magic of AES 2024 conference. You'll kind of feel like your presence was there. And of course I'll share at least one key lesson per lecture. As well as some interviews from the amazing Protruserati and mentors that I met. Like, I shook hands with John Kois and Frank Spear within an hour, and I didn't even wash my hands. So how did this trip begin? Well, it began with what we call level one permission. Now I joke about this level one permission whenever we organize a trip like last year, a few years ago Porto, this year we're going Sicily in June for Vertipreps and of course for Chicago, I needed level one permission. So people ask me on Facebook, what is level one permission? Okay, level one permission is when you have to ask your spouse if you can go. I know that sounds really weak, but it's a reality when you're a father of two boys, young boys, I had to absolutely beg Sim to let me go to Chicago. I'm so thankful, me and Mahmoud were both thankful to our wives for letting us go.

Now I know we are well into March at the time of publishing and we're still stuck on adhesive month, which was February. March is supposed to be documentation month, but don't worry. I've got some cool stuff coming to catch up in March, including what software I'm using at the moment to write my notes for me. Jaz's Introduction:I know it sounds crazy. I have two, I have two dreams in my life. One is to never have to wash dishes ever again. And no, I don't have a dishwasher. My kitchen is not big enough at the moment. And number two is to never have to write my notes again. So, you know, dishwasher can wait, but number two is actually almost happening. And I've been on this crazy journey to discover the best software or the best way. So as part of documentation month, I'll make it up to you by the end of this month to reveal which softwares I've tried and tested and which one I've decided was the best. This episode, of course, is the part two following the discussion that we had on the Bioclare Adhesive Philosophy. Like, I don't want this two part episode to be about the Matrix. Like, the Matrix is awesome. Like, we know that Bioclare matrices, they do very well, but I'm not here to shine a light on, on just the Matrix.
