In more than a decade of training and clinical practice as a psychiatrist, I've become increasingly disillusioned by the American healthcare system. It prioritizes profit over progress at the expense of OUR health. The average American spends almost twice as much on healthcare as the next highest spending country, yet our life expectancy drags four years behind other industrialized nations and ranks OUTSIDE the top 50 in the world. The system is a disaster, yet it's broadcast as 'the greatest healthcare system in the world.' Renegade Psych wants to return medicine's focus to PROGRESS OVER PROFIT, and limit the influence and power of the pharmaceutical industry and big business entities, whose primary motivations are financial in nature.

This conversational podcast is released monthly via SERIES on particular topics. There are 8-12 SEGMENTS, or episodes, per topic. I interview internationally-recognized experts in their fields AND relatively unknown, up-and-coming healthcare providers. Hopefully, we can educate patients AND providers about our medical system (especially mental health), so they can make better and more informed healthcare decisions.

In this segment, Jenny and I discuss our cases involving first episode of psychosis in menopause, as well as our differential diagnosis for this phenomenon.  While there simply isn't enough research on this specific presentation, it seems to be common sense that trying hormones to alleviate symptoms would be a much safer and possibly more effective alternative to antipsychotics, antidepressants, or even shock therapy.  Knowing that female patients with known severe mental conditions like schizophrenia and bipolar disorder have worsening of their symptoms during transitional periods like menopause indicates a potential role for a hormone therapy trial.  Hope you enjoy. Clozapine is one of the first antipsychotics with a unique chemical structure compared to any other antipsychotic.  It is the most effective at treating patients with treatment-resistant schizophrenia, but is severely underutilized despite its efficacy.  It does have a significant side effect profile that needs to be monitored and managed, but can provide relief to patients who have not found relief from other medications.  Today, I bring on Jose Rubio, MD, at Zucker Hillside Hospital, who specializes in treatment-resistant schizophrenia and is an expert on using Clozapine.  He also trained under John Kane, the original MD and researcher who was tasked by the FDA, which was trying to ban Clozapine due to a string of Scandinavian deaths related to a rare side effect, to bring inpatient treatment-resistant schizophrenic patients in the 1970s OFF of clozapine, and came back to the FDA saying that he wasn't able to find effective treatment for the majority of them and petitioned the FDA to reconsider their ban due to Clozapine being so effective.  Clozapine is very cheap and is another example of us prioritizing less effective and more expensive treatments over tried-and-true treatments.  It DOES have a severe side effect of agranulocytosis (along with a couple others), which impairs the person's immune system to fight off infections, so it requires a significant amount of monitoring especially early in the treatment.  But, for the vast majority of patients, Clozapine is not only tolerated, but provides them relief from their symptoms and a chance to live a more normal life.  Hope you enjoy! This is the introductory episode to our opening series on Antidepressants - Going Backwards with Dr. Ken Gillman.  We'll talk about how 1st line antidepressant medications have become less effective with time, how the diagnostic criteria for depression have been watered down, and reveal how Evidence-Based Medicine can be heavily manipulated.  We'll also give advice on how to best recognize subtypes of depression and treat them accordingly.  Dr. Gillman is an internationally well-known and well-recognized expert on diagnosing biologic depression and using MAOIs, a class of antidepressants widely used in the 1960s and 1970s but rarely used today, and is also one of, if not THE, leading expert on serotonin toxicity (improperly referred to as 'serotonin syndrome' by non-experts) and runs a website called psychotropical.com trying to shed light on these issues, as well as the historical and current problems with psychiatric and medical research.
