On this episode of The Sonography Lounge hosts Lori and Trisha sit down with Dr. Charlotte Derr & Dr. Allyson Hansen to discuss the GUARD Protocol (Gastric Ultrasound for Aspiration Risk Determination).
With the increased use of GLP-1 Agonists (Semaglutide, with brand names such as Ozempic, Wegovy, Rybelsus, Etc) for both diabetes and weight loss, there has been an increased number of aspirations in patients undergoing procedures. One of the effects of GLP-1 agonists is delayed gastric emptying which keeps patients feeling "full" thus reducing the desire to eat and overall caloric intake and blood sugar variations. Typically 6-8 hours not eating is enough time for a patient about to undergo an elective procedure to digest and empty their stomachs thus reducing the risk of aspiration while under anesthesia. With patients on GLP-1 agonists, we are seeing gastric contents remaining for several days after the last meal which can increase the risk of aspiration.
Dr. Derr and Dr. Hansen, discuss how they are using ultrasound and the GUARD protocol to determine the volume of food in a patient's stomach before elective and emergency procedures. This quick and simple bedside technique can drastically alter a provider's pre-procedure course of action.
CHARLOTTE DERR, MD, RDMS, FPD-AEMUS, FACEP
Dr. Derr is the Professor of Emergency Medicine and the Fellowship Director of the Advanced Emergency Medicine Ultrasound Fellowship Program at the University of South Florida Morsani College of Medicine in Tampa, FL and serves as the Co-Medical Director at Gulfcoast Ultrasound Institute.
She is a fellow of the American College of Emergency Physicians and is a registered diagnostic medical sonographer. She has also obtained the Focused Practice Designation in Advanced Emergency Medicine US from the ABEM.
She has authored and co-authored numerous articles, book chapters, and other publications on ultrasound and lectures throughout the US and internationally on diagnostic ultrasound.
ALLYSON HANSEN, DO
Dr. Allyson Hansen is the Ultrasound Director at the University of South Florida Emergency Medicine program. She received her education at the University of South Florida and Campbell University. Dr. Hansen completed her Hackensack University Medical Center residency and Emergency Ultrasound Fellowship at Tampa General Hospital. She is passionate about teaching and is an instructor at Gulfcoast Ultrasound Institute.

JAMES MATEER, MD, RDMS
Dr. James Mateer was one of the pioneers in Emergency Medicine Ultrasound applications and was among the first Emergency Physicians to obtain their registry for Diagnostic Medical Sonography from the ARDMS.
Dr. Mateer has served as the Medical Director of Gulfcoast Ultrasound Institute since 1996. He is co-editor for the textbook: “Emergency Ultrasound” and has authored numerous chapters and articles on Emergency Medicine and Emergency Ultrasound applications. He has lectured extensively throughout the United States and Internationally.

David P. Bahner, MD FAAEM, FACEP, FAIUM
Dr. Bahner is Professor and the Ultrasound Division Director Department of Emergency Medicine at The Ohio State University Medical Center, Columbus, OH. He has specialty training in hyperbaric medicine, emergency medicine research and an advanced teaching fellowship with ACEP. He was Membership Chair, AIUM and served on the Board of Directors, at the Society of Ultrasound in Medical Education. He is currently the AIUM delegate to the AMA and an ex oficio member of the AIUM Board of Governors.
He has earned fellow status as faculty of AAEM, ACEP and AIUM. He has multiple publications, written book chapters, letters and inventions as an advocate for point of care ultrasound. Dr. Bahner has published more than 60 articles on point of care ultrasound and education. He has lectured internationally and nationally on this subject, and continues to champion clinician performed ultrasound for better patient care and patient safety as the Ultrasound Division Director within the OSU Department of Emergency Medicine.
Andrew Laudenbach, MD
Dr. Laudenbach is the Assistant Director of Emergency Ultrasound and Co-Director of the Emergency Ultrasound Fellowship program at Hennepin Healthcare in Minneapolis. He is an assistant Professor of Emergency Medicine at the University of Minnesota Medical School.
He is an editor for the Ma & Mateer Emergency Ultrasound textbook, and has authored several articles and textbook chapters on point of care ultrasound.

Dr. John Broussard, (https://www.thealliancerx.com/) is board certified in Family Medicine and Sports Medicine and specializes in both regenerative and osteopathic manipulative medicine. He earned his Doctor of Osteopathic Medicine degree from Nova Southeastern University in South Florida and completed his residency training in Family Medicine at an osteopathic hospital, practicing in the urgent care setting. He then followed to complete a Sports Medicine Fellowship at the Institute for Non-Surgical Orthopedics in Florida. In 2014, he integrated ultrasound into his practice for diagnostics and guided interventional procedures. In 2020, he and his brother opened their own practice right here in sunny St. Pete. Together, they help active adults and athletes quickly overcome injury and get back to the activities they love.
Dr. David Wang, (https://www.rosm.org/) Who earned his Doctor of Osteopathic Medicine degree from The West Virginia School of Osteopathic Medicine, where he was selected as a teaching Fellow in Osteopathic Manipulative Medicine. He completed his residency training in Physical Medicine and Rehabilitation at Harvard Medical School/Spaulding Rehabilitation Hospital, during which time he was appointed Chief Resident. He is a founding partner and Director of Education and Training of Regenerative Orthopedics and Sports Medicine, with offices throughout the Metropolitan DC area. He specializes in diagnostic musculoskeletal ultrasound, non-fluoroscopic interventional spine, osteopathic manual treatment, and regenerative medicine including prolotherapy, PRP, orthobiologics, and lipoaspirate stem cell therapies.
Dr. Imran Siddiqui, (https://www.rosm.org/) Currently serves as a Partner and the Director of Clinical Operations at Regenerative Orthopedics and Sports Medicine in Washington, DC. He graduated medical school Cum Laude from Thomas Jefferson University where he was inducted in to the Alpha Omega Alpha honor society. After which, he completed his residency at Harvard Medical School and Spaulding Rehabilitation Hospital where he trained extensively in musculoskeletal ultrasound and regenerative medicine and was awarded resident teacher of the year. Currently, Dr. Siddiqui also servers as an ultrasound and instructor and board member for the Interventional Orthobiologic Foundation. He also has instructed at Harvard Medical School, Orthosound Educational Institute, and the American Academy of PM&R.

The Sonography Lounge is proud to have with us, Dr. Michael Prats, founder of the Ultrasound G.E.L (Gathering Evidence through Literature) podcast which takes various medical journals and research publications that focus on ultrasound and breaks them down into an easy to understand digestible format.
Dr. Prats is currently the Assistant Ultrasound Director and Director of Ultrasound Research in the Division of Ultrasound in the Department of Emergency Medicine at the Ohio State University. He completed his undergraduate studies at the University of Pittsburgh, with dual majors of neuroscience and philosophy, before attending medical school at the Ohio State University. He subsequently completed his residency in emergency medicine at Drexel University College of Medicine in Philadelphia, followed by an emergency ultrasound fellowship at Ohio State University.

In 2020 he added brick and mortar services to the existing Medicare Part B house call practice. Additionally, As of 2020 after being exposed to diagnostic musculoskeletal ultrasound Dr. Charles sought training through WCUI School of Medical Imaging with instructor Jeannine Noble PT, RMSK and also pursued training through Gulfcoast Ultrasound Institute in August 2022. In January 2023, Dr. Charles passed a board certification in diagnostic musculoskeletal ultrasound to earn the RMSK credential which is the highest level of proficiency in diagnostic musculoskeletal ultrasound

The Sonography lounge is happy to be joined by Dr. Jon Jacobson and Dr. David Wang, two wild and crazy guys and masters of musculoskeletal ultrasound who are leading the way in this consistently growing industry. This episode is taking place as part of our Happy Hour series, winding down from a long day with a drink, casual banter, and questions to get to know each other.
Dr. Jacobson is a Musculoskeletal Radiologist with Lenox Hill Radiology in New York, NY.
Dr. Jacobson earned his RMSK credential in 2012 from the American Registry of Diagnostic Medical Sonography. He has published over 250 manuscript publications and has authored numerous book chapters as well as completing his 3rd edition of the textbook “Fundamentals of Musculoskeletal Ultrasound.” He lectures extensively throughout the United States and internationally and we have had the pleasure of working with him for many years.
Dr. David Wang earned his Doctor of Osteopathic Medicine degree from The West Virginia School of Osteopathic Medicine, where he was selected as a teaching Fellow in Osteopathic Manipulative Medicine. He completed his residency training in Physical Medicine and Rehabilitation at Harvard Medical School/Spaulding Rehabilitation Hospital, during which time he was appointed Chief Resident.
He is a founding partner and Director of Education and Training of Regenerative Orthopedics and Sports Medicine, with offices throughout the Metropolitan DC area. He specializes in diagnostic musculoskeletal ultrasound, non-fluoroscopic interventional spine, osteopathic manual treatment, and regenerative medicine including prolotherapy, PRP, orthobiologics, and lipoaspirate stem cell therapies.
Dr. Wang has been actively teaching regenerative medicine nationally and internationally since 2008, including over 20 intensive prolotherapy training medical missions in Honduras, Mexico and Greece.

The Sonography Lounge sits down with The American Institute of Ultrasound In Medicine (AIUM) and Peter Magnuson, CAE, who currently serves as the Director of Communications and Member Services. In this role, he oversees membership, marketing, social media and the Journal of Ultrasound in Medicine. Peter has been at the AIUM for 8 years and enjoys developing new programs and opportunities for the AIUM’s diverse membership.
Peter goes over some exciting grant opportunities available to healthcare providers interested in furthering their ultrasound education. Multiple $500 grants will be awarded this round (submissions are due December 16, 2022). The Sonography lounge hosts, Lori Green and Trisha Reo Sit down with the founders of Focused Ultrasound Resources (FUR) Emily Downs, M.Ed., RDMS, RVT, RDCS, RT, and Michelle Sima, MS, RDMS, RVT to talk about the veterinary ultrasound market and opportunities within this exciting and evolving field.
Emily Downs M.Ed., RDMS, RVT, RDCS, RT
Emily graduated from the University of Nebraska Medical Center with a BS in Radiation Sciences and subsequently graduated from the University of Nebraska - Lincoln with a Master’s in Education. She served as the Program Director, and is a proud alumnus of the Diagnostic Medical Sonography program at the University of Colorado Hospital in Denver, CO. She provided lectures for all areas of the ultrasound program curriculum and as a human sonographer, performs general, high risk obstetric and fetal echocardiography ultrasound. Emily is an active member of Society of Diagnostic Medical Sonographers and has several publications. She has also served on the board of directors for the International Foundation for Sonography Educators and Research (IFSER) and also works with the Pan American Health Organization (WHO) as a temporary advisor and international educator for ultrasound education.
With a love for animals, Emily transitioned to veterinary sonography several years ago and co-founded Focused Ultrasound Resources (FUR). She has developed educational software for veterinary ultrasound through her company, Rad Imaging Skills and currently performs small animal ultrasound throughout the Denver area.
Michelle Sima MS, RDMS, RVT
Michelle graduated from Texas A&M University with her Bachelor of Science in Biology and consequently a Master’s at Colorado State University. During her Master’s, she worked on research with the veterinary radiologist using different imaging modalities, including ultrasound, to diagnose pneumonia in rabbits. This was her initial discovery of her love for ultrasound, so she pursued her career in ultrasound at the University of Colorado Hospital. She has worked at University of Colorado, outpatient imaging facilities, and as an ultrasound applications specialist. She has been a clinical instructor for three programs and is an active member of the ARDMS, SDMS, and AIUM. She has served on the ARDMS board writing panels, and is a published author of several educational courses through the SDMS.
With her goal to get back into the veterinary field, Michelle began working with Antech Sound Imaging as a Clinical Education Specialist II, training veterinarians, veterinary technicians, and veterinary radiologists about ultrasound. She has taught in private and corporate veterinary hospitals, as well as at symposiums, conferences and private education events. Michelle has been a recognized speaker at veterinary conferences including VMX and WVC. During her time at Antech Sound Imaging she realized a need for professionals performing veterinary ultrasounds in the field. She then co-founded Focused Ultrasound Resources (FUR) to address the community’s needs.

Alexander B. Levitov, MD, FCCM, FCCP, RDCS
Dr. Levitov is a Professor of Medicine at East Virginia Medical School in Norfolk, VA. He is a registered cardiac ultrasound sonographer, NBE certified and a Fellow of American Society Critical Care Medicine and American College of Chest Physicians.
Dr. Levitov authored and co-authored several books including Critical Care Ultrasonography and Bedside Ultrasonography in Clinical Medicine, multiple book chapters and numerous publications. He lectures extensively in the United States and internationally. Dr. Levitov is 1 of 4 people in the US to receive the Healthcare Hero Award in 2016.
David P. Bahner, MD FAAEM, FACEP, FAIUM
Dr. Bahner is Professor and the Ultrasound Division Director Department of Emergency Medicine at The Ohio State University Medical Center, Columbus, OH. He has specialty training in hyperbaric medicine, emergency medicine research and an advanced teaching fellowship with ACEP. He was Membership Chair, AIUM and served on the Board of Directors, at the Society of Ultrasound in Medical Education. He is currently the AIUM delegate to the AMA and an ex oficio member of the AIUM Board of Governors.
He has earned fellow status as faculty of AAEM, ACEP and AIUM. He has multiple publications, written book chapters, letters and inventions as an advocate for point of care ultrasound. Dr. Bahner has published more than 60 articles on point of care ultrasound and education. He has lectured internationally and nationally on this subject, and continues to champion clinician performed ultrasound for better patient care and patient safety as the Ultrasound Division Director within the OSU Department of Emergency Medicine.

The Sonography Lounge Hosts Lori and Trisha sit down with Dr. John Broussard to talk about the use of ultrasound in conjunction with the EmSculpt Neo and EmSella technology to track fat loss, muscle gain, and the treatment of incontinence.
Dr. John Broussard is board certified in Family Medicine and Sports Medicine and specializes in both regenerative and osteopathic manipulative medicine. He earned his Doctor of Osteopathic Medicine degree from Nova Southeastern University in South Florida and completed his residency training in Family Medicine at an osteopathic hospital, practicing in the urgent care setting. He then followed to complete a Sports Medicine Fellowship at the Institute for Non-Surgical Orthopedics in Florida. In 2014, he integrated ultrasound into his practice for diagnostics and guided interventional procedures.
Dr. Broussard is the co-founder of Alliance Regen and Rehab in downtown St Petersburg, FL. In 2020 he and his brother opened their sports medicine practice to provide effective solutions to athletes and active adults to quickly overcome injuries and get back to the activities they love.”
Dr. Broussard has provided skills-based labs, supervised residents and medical students in osteopathic and sports medicine clinical settings, and presented at state conferences. He believes in using integrative medicine to help patients receive comprehensive care in one setting.
