Some of these advancements include the incorporation of more lymphatic massage techniques, the introduction of NanoVi technology to improve protein folding and recovery, and the use of H-Wave therapy to further enhance lymphatic drainage and aid in patient recovery. For today, however, let’s focus on fat transfers. Drawing from my extensive experience with fat transfers, a technique I’ve refined over many years, I explain that fat transfers are effective because they use the patient’s own tissue, making them autologous and thus free from the risks associated with donor tissues, such as genetic mismatches. Fat transfers are versatile, as adipose-derived stem cells can differentiate into fat cells, bone cells, or cartilage, providing a range of benefits. My initial interest in this technique stemmed from a personal desire to regenerate cartilage in my knees after multiple surgeries, leading to a deep understanding of the procedure’s potential. With extensive experience in sarcoma reconstruction, I can confidently say that fat transfers are safe when performed correctly. It's essential not to transfer fat to areas with existing tumors or sarcomas, as these mesenchymal regions could complicate the procedure. For breast augmentation and reconstruction, fat transfers have been effective for many years, and I have conducted numerous procedures since 2005. Successful outcomes depend on transferring a mixture of adipocytes, vascular cells, and adipose-derived stem cells. This technique requires precise placement of fat in specific areas to avoid complications and ensure proper integration with the surrounding tissue. There were early safety concerns regarding fat transfers for cosmetic purposes, such as buttock augmentation, due to technical issues when the procedure first gained popularity in South America and Miami. However, with advancements in techniques and improved safety measures, fat transfers for both buttock and breast augmentation have become reliable and effective procedures. Fat transfer for breast implant illness, including stem cells and healing When the skin gets really tense, you'll notice a condition called "peau d'orange," where the surface of the skin starts to look like the surface of an orange. This indicates too much pressure and that expansion should stop. Unlike implants, which can hold their shape and maintain more volume, fat cannot be continuously added. The amount of fat that can be added depends on the initial condition of the skin. For younger clients with tight skin, multiple sessions may be needed to gradually increase volume. In cases where I remove implants that have been in place for a long time, often after weight changes, pregnancies, or breastfeeding, the skin is usually stretched out. After removing the implant with its capsular material intact, I don't fill the old implant pocket and instead, place the fat beneath the skin, where it belongs, and let it heal. Adipose-derived stem cells in the fat offer numerous benefits, aiding in wound rejuvenation and healing. They can help turn genes on, produce exosomes, modulate immune responses, and improve mitochondrial function. This is why fat transfers are more than just about the fat itself – they bring substantial healing properties. We've integrated PCR testing of scar capsules into our practice which examines DNA fragments and this approach has yielded great results. Dr. Blodgett highlights the significance of understanding biofilm formation, particularly concerning teeth that no longer have a blood supply due to procedures like root canals. In an upcoming series based on 900 consecutive samples, we found that approximately 30 to 34% exhibited biofilm colonization, consisting of various microorganisms like bacteria, fungi, and mycobacteria. This microbial presence within the roots suggests a significant deviation from what's considered normal. Despite efforts to ensure quality testing by sending samples to a research-based laboratory specializing in PCR testing, there remain limitations in detecting microscopic processes through macroscopic examinations like CT scans, MRIs, or regular X-rays. Furthermore, Dr. Blodgett emphasizes the interconnectedness of oral health with overall well-being, noting how disruptions in the oral microbiome can contribute to various systemic problems. Issues such as poor digestion, irregular bowel movements, and compromised nutrient absorption can stem from an imbalanced microbiome, impacting the body's ability to heal and function optimally. Oral health and hygiene, including protein intake and its impact on teeth Maintaining normal digestive processes is crucial for overall health, as it impacts our gut microbiome and downstream health outcomes. For instance, missing teeth can hinder the chewing process, affecting how well we break down food and utilize nutrients. Common practices, like consuming acidic beverages or having multiple metal fillings, can disrupt the delicate balance of the mouth's ecology. It is therefore necessary to address these issues proactively rather than turning a blind eye. Addressing these issues involves considering the energetic and biological implications of dental treatments and materials used, as well as acknowledging potential sensitivities individuals may have to certain substances. By adopting a more holistic approach to oral care, we can better support both oral and systemic health. Oral health and its impact on overall health, including the importance of proper dental care and the risks of poor oral hygiene Many individuals are transitioning towards a more natural and holistic approach to well-being, especially those who have undergone procedures like breast augmentation or reconstruction and are now considering removing breast implants. In my practice, I emphasize the importance of dietary adjustments, including cutting out gluten, dairy, and processed sugars, to support overall health and well-being. These dietary changes are crucial as sugar, in particular, can be highly addictive and detrimental to health, impacting brain function and energy levels. That is why I always encourage clients to maintain a balanced fluid intake, neither overly acidic nor alkaline. Although I don't have a specific diet regimen, I advocate for a higher protein intake tailored to individual needs, typically ranging from one and a half to two and a half grams of protein per kilogram of body weight per day. This adjustment may require creative solutions for those struggling to meet their protein goals, such as incorporating amino acid supplements or protein-rich smoothies. In terms of oral health, dietary adjustments and fluid intake can have a significant impact. For example, high-protein diets may lead to buildup on teeth surfaces, as observed during teeth cleaning sessions. While protein intake remains essential for optimal functioning, managing its effects on oral health requires attention to hygiene and possibly adjustments in dental care routines. In surgeries, dentists often check for dental problems. If there's an issue, like infection or inflammation, the surgery might be postponed due to the risk of bacterial spread and potential complications like endocarditis. However, the sad truth is that despite longstanding awareness of these risks, they're not always taken seriously. This shows the need for greater emphasis on oral health in overall health management. Dental implants and cancer risk, with a focus on root canals and lymphatic drainage The standards set by organizations like the American Heart Association highlight the risks associated with microbial exposure in dental procedures. Surgeries involving heart valves, knee replacements, and hip joints pose risks because they share the same lymphatic drainage and blood supply pathways as the mouth. Despite this knowledge, the potential risks of microbial exposure from procedures like root canals are often overlooked. While we can’t stop the time to avoid how our faces change, we can certainly influence how our faces age. In this episode, we dive deep into the interplay of genetics, skincare treatments, and lifestyle, providing a roadmap for those who seek to take charge of their aging process. The foundation of the aging process When it comes to the topic of changing how your face ages, there are several key factors to consider before even delving into the realm of skincare as well as aspects beyond our control. These elements are the foundation of our aging process. It's surprising how little attention we give to the role of bones in the aging process, but the truth is that our bones also undergo resorption, a natural thinning process. Genetics plays a significant role in determining how much resorption an individual may experience, and this can even impact their susceptibility to conditions like osteoporosis. Another often overlooked aspect is the role of muscle in facial aging. While it's not a frequently discussed topic, muscle changes, especially around the eyes, can play a significant role in the aging process. In some cases, muscles may have hypertrophy, which can contribute to the overall transformation of one's appearance. So, when it comes to changing how our face ages forever, it's essential to consider these factors that extend far beyond just skincare products and routines. Expressive facial expressions also lead to more lines, muscular hypertrophy in the jaw, and the loss of specific fat compartments in the face, resulting in deflation and puffy eyes. Skin changes are thus prominent, manifesting as lines, wrinkles, folds, and sagging. In addressing these concerns, treatments like HydraFacial can temporarily enhance turgor/rigidity and fluid retention in the skin, providing a smoother and tighter appearance for special occasions. Genetics, diet, and supplements for skin health and overall wellness I would like to emphasize genetics in your skincare plans, particularly concerning detox pathways, hormone metabolism, and sleep. Understanding your genetic predispositions can be a game-changer in tailoring a personalized approach to facial aging. It allows you to make more informed decisions and select treatments and products that align with your unique genetic makeup, enabling a more effective and sustainable way to alter the course of how your face ages. I also recommend using air filters and avoiding plastic for drinking to ensure skin health and overall wellness. When it comes to diet, make sure to cut out gluten, dairy, processed foods, and unnatural sugars to enhance overall wellness before any procedure. Additionally, when setting up a skincare routine, ensure that the skincare products suit your specific needs, which may include ingredients for reducing pigmentation, increasing collagen production, or targeting fat reduction. By incorporating these tailored skincare products into your routine, you're not only proactively addressing the aging process but also investing in the long-term health and vibrancy of your skin. Treatments to improve skin health Skin care plays a crucial role in tightening the skin and promoting collagen synthesis. Having the right cleanser and moisturizer tailored to one's skin type is essential, as is using sun protection, which are basic principles of skincare. I utilize products like elastin and my personal line of products is designed to stimulate collagen production. Daily use of these products can help improve skin tightness, so make sure to grab yours now! The muscular blocking agents that we use assist in reducing the appearance of fine lines. These agents are commonly applied to areas like the forehead, crow's feet, and laugh lines around the nose, as these wrinkles result from muscular contractions. There are various options for these muscular blocking agents, including Botox, Dysport, Xeomin, and Taxify, each with unique properties and compositions. The next step involves volumizing agents, commonly known as fillers. I prefer using fillers once the desired level of skin tightness has been achieved. I'm cautious about over-volumizing and tend to lean toward hyaluronic acid fillers due to their reversibility. Using reversible options is essential, especially for new patients who might want a subtle change. Hyaluronic acid fillers, which have been around since 2005, not only add volume but also stimulate collagen synthesis. A newer hyaluronic acid filler on the market is Revanesse Versa, known for its unique chemical structure that allows it to stretch when the face animates, providing a natural look. Another option for volumization is fat, but I use it sparingly in facial treatments, as it's not reversible and is often more suitable for procedures like mid-face lifts. In addition to these fillers, we also utilize the patient's own stem cells for injections, contributing to age reversal. As for non-invasive treatments, I offer different options in my practice. One involves focused ultrasound, specifically Sofwave™, which is FDA-cleared and provides non-invasive facial contouring, even around the eyes and cheeks. We've found it effective, and it's a less invasive alternative. Additionally, we have a non-invasive solution called Evoke, by InMode, which can treat the cheeks, jawline, and chin. Evoke employs bipolar radiofrequency technology, and the choice between these technologies depends on patient selection and their baseline level of skin laxity. As Candice recovers, her challenges continue. Severe adhesions send her to a doctor for scar release sessions, but incorrect practice of the technique causes the scars to tether back down. Knowing a fat transfer is on the horizon, she searches for the least invasive solution as she has several surgeries under her belt. She finds a doctor who performs non-invasive fat transfers, but he injects way more fat than needed, causing her breasts to swell up and become larger than her implants ever were. When she develops a fever, she makes an emergency trip to California to return to the fat transfer surgeon, which ends with liquidated fat leaking out of her breasts. While searching for the best surgeon to first believe her and then remove her breast implants, Candice relied on Yelp reviews and Facebook groups and met with numerous surgeons. Most would agree to take her implants out, but dismissed the possibility that her implants were causing her symptoms. After finding a qualified surgeon who listened to her and understood the importance of removing the entire capsule, she moved forward with a total capsulectomy. Recovery was rough, with an infection caused by drains followed by a fever and a pretty miserable few days. Contrary to common practice, studies show that the use of drains is associated with a higher rate of infection. After many years of research, detoxing on her own, and testing for anything and everything she could think of, Candice finally realized her chronic illnesses were breast implant-related and decided to explant. Within a few months of her first breast augmentation in 2003, Candice Michele Barley began experiencing chronic fatigue. Although she had young children, this was far beyond the usual new mom exhaustion and her doctor dismissed her concerns. The first implant rupture occurred two years later while doing yard work. She had both implants replaced and like many women do when they get a new set, she went a little larger. Almost right away, restless leg syndrome began keeping her awake at night. Two years later while riding a roller coaster at her son’s birthday party, another implant ruptured and sent her back to the operating room for another double replacement. After this surgery, heart palpitations and unusual rashes appeared. Still no one considered these problems could be linked to her breast implants. In 2011, Candice had her 4th breast surgery, swapping saline for the new silicone gel “gummy bear” implants. This was when things really took a turn for the worse.
