He leads efforts towards value-based care, health equity, and delivery system transformation in Medicare, which covers 63 million Americans and spends over 20% of US healthcare dollars. He was the Chief Medical Officer and Chief Innovation Officer for Pennsylvania's Department of Human Services, overseeing the state's Medicaid program. Dr. Jacobs is a board-certified Internal Medicine Physician who continues to see patients during the pandemic. He was chosen by the governor to lead Whole-Person Health Reform and created Pennsylvania's first equity incentive program and a program to address social determinants of health.
