Description

The Male Fertility Podcast is the UK’s leading show dedicated to male fertility, hosted by Ciaran and Shaun, two men who’ve lived the reality of infertility diagnoses, fertility treatment, and everything that comes with it.

Whether you’re dealing with low sperm count, poor motility, low morphology, azoospermia, or a varicocele diagnosis, this podcast is the resource you wish you’d had from day one.

Each episode covers what actually matters: IVF and ICSI from the male perspective, navigating NHS referrals and waiting lists, the real cost of private treatment in the UK, lifestyle changes that can improve sperm quality, and the emotional and mental health impact that often goes unspoken.

We speak to fertility specialists, urologists, andrologists, and men sharing real, unfiltered stories — from difficult diagnoses through to successful pregnancies.

Topics we cover:

Male factor infertility: diagnosis and treatment options
Sperm health: count, motility, morphology, DNA fragmentation
IVF, ICSI, MESA, TESE, micro-TESE
Varicocele surgery and recovery
Supplements, diet, exercise, and lifestyle changes
Mental health, relationships, and coping strategies
NHS vs private fertility treatment in the UK
Costs, funding, and access to treatment
Supporting your partner
Real stories and success journeys

This podcast exists because male fertility is too often overlooked. Half of all infertility involves a male factor, yet most support is aimed elsewhere. We’re changing that.

If you’re facing fertility challenges, or supporting someone who is, subscribe now — you’re not alone.

Can you have a normal semen analysis but still experience problems with fertility? And could sperm DNA fragmentation help explain why some couples struggle to conceive, experience recurrent miscarriage or have unsuccessful fertility treatment? In this episode of The Male Fertility Podcast, we welcome consultant urologist Jonathan Ramsay for an in-depth conversation about sperm DNA fragmentation, male fertility testing and what might be happening beyond the standard semen analysis. This is the second episode in our special DNA fragmentation mini-series, in partnership with Examen Lab.Jonathan has spent decades working in male fertility and has been investigating sperm DNA fragmentation since the early development of testing. He shares his clinical experience of men who have normal or near-normal semen analysis results but whose fertility outcomes suggest there may be more to investigate. We explore the difference between single-strand and double-strand DNA breaks, why this distinction matters, and what emerging research might tell us about sperm quality and fertility outcomes. We also discuss when DNA fragmentation testing may be worth considering, including recurrent early pregnancy loss, unsuccessful ICSI cycles, poor embryo development and situations where the clinical picture doesn't quite add up.But this conversation goes beyond testing.Jonathan explains how lifestyle, nutrition, infection, varicoceles, exercise intensity and overheating may influence sperm quality. We also discuss the importance of interpreting test results in context, rather than treating one result as a definitive answer. One of the biggest themes in this conversation is the need to understand why fertility treatment fails, not simply how to improve the next outcome. Jonathan explains why better research, greater collaboration between male and female fertility specialists, and a more holistic approach to reproductive health are needed.IN THIS EPISODE, WE DISCUSS:Why a normal semen analysis doesn't necessarily mean sperm quality is normal. How ICSI changed the landscape of male fertility treatment and investigation. What sperm DNA fragmentation is and how it is measured. The difference between single-strand and double-strand DNA breaks. Why double-strand DNA damage is attracting increasing research interest. When DNA fragmentation testing may be worth discussing with a clinician. The potential relevance of DNA fragmentation in recurrent miscarriage and unsuccessful ICSI.The relationship between varicoceles, infection, lifestyle and sperm DNA quality. How exercise, cycling, overheating and extreme temperatures may affect sperm health. Why DNA fragmentation results aren't always straightforward to interpret.Whether DNA fragmentation testing could become more routine alongside semen analysis. The importance of investigating unexplained infertility and understanding why treatment fails. The future of male fertility research and the need for a more joined-up approach to fertility care.KEY QUESTIONS ANSWEREDCan you have normal semen analysis results but high sperm DNA fragmentation?Yes. Jonathan explains that men can have normal or near-normal semen parameters while still showing abnormal sperm DNA fragmentation. This is one reason why semen analysis may not tell the whole story. When should you consider a sperm DNA fragmentation test?Jonathan discusses situations where further investigation may be worth considering, including repeated early miscarriages, unsuccessful ICSI cycles, poor embryo development or when fertility outcomes don't match what might be expected from the initial investigations. What is the difference between single-strand and double-strand DNA fragmentation?Single-strand breaks affect one strand of the DNA structure, while double-strand breaks affect both. Jonathan discusses why double-strand damage may have different implications for fertility, while emphasising that the research is still developing. Can lifestyle changes improve sperm DNA fragmentation?Lifestyle, nutrition, infection, exercise and heat exposure are all discussed as potential factors. Jonathan shares observations from his clinical experience, while highlighting the importance of looking at the individual rather than assuming one approach works for everyone. Will DNA fragmentation testing become part of routine male fertility assessment?Jonathan believes it could become more widely used alongside semen analysis, but further research and clinical data are needed.EPISODE TIMESTAMPS00:00 The Importance of Openness in Fertility Discussions02:49 Introduction to Male Fertility and the Guest06:08 The Journey into Male Fertility09:07 Understanding DNA Fragmentation in Male Fertility11:54 The Role of Lifestyle in Sperm Quality14:53 The Evolution of DNA Fragmentation Testing17:50 Challenges in DNA Fragmentation Research20:47 Selecting Sperm Without DNA Damage23:45 The Future of Male Fertility Research35:51 The Challenge of Acknowledging Mistakes in Research38:38 Understanding DNA Fragmentation Tests42:02 Interpreting DNA Fragmentation Results45:10 Lifestyle Factors and Their Impact on Fertility49:03 The Relationship Between Semen Analysis and DNA Fragmentation54:39 Exploring the Reasons Behind Fertility Issues01:00:00 The Future of DNA Fragmentation Testing in Fertility01:07:23 Integrating Male and Female Perspectives in Fertility CareABOUT OUR GUEST: JONATHAN RAMSAYJonathan Ramsay is a consultant urologist with decades of experience in male fertility and reproductive medicine. His work has included investigating the relationship between sperm quality, DNA fragmentation and fertility outcomes, helping to develop a greater understanding of the male contribution to fertility. In this episode, Jonathan shares his clinical experience and thoughts on the future of male fertility investigation and research.EPISODE PARTNER: EXAMEN LABThis episode is produced in partnership with Examen Lab, supporting greater awareness and understanding of sperm DNA fragmentation testing.Find out more:Website: https://examenlab.com/Partner clinics: https://examenlab.com/hubs-partner-clinics/ABOUT THE MALE FERTILITY PODCASTThe Male Fertility Podcast is here to make male fertility easier to understand, bringing together experts, clinicians, researchers and real-life experiences to give men the information and support they need.We talk about sperm health, male infertility, lifestyle, nutrition, exercise, reproductive health and the emotional impact of fertility challenges. Our aim is simple: to get men talking, improve understanding and help men take a more active role in their fertility journey.Visit The Male Fertility Hub: https://www.themalefertilityhub.com/Follow us on Instagram: @themalefertilitypodcastEmail: [email protected] you found this episode useful, please follow the podcast, leave a review and share it with someone who might benefit from the conversation.Disclaimer: This episode is for educational and informational purposes only. Sperm DNA Fragmentation: What You Need to Know | Lalana SongraWhat is sperm DNA fragmentation, why does it matter for male fertility, and what can a DNA fragmentation test actually tell you?In this episode of The Male Fertility Podcast, we kick off a new mini-series exploring sperm DNA fragmentation, in partnership with Examen Lab.We’re joined by Lalana Songra, Chief Medical Advisor at Examen Lab, to break down the science behind sperm DNA fragmentation and explain what men and couples need to know about sperm DNA damage, testing and sperm health. We start with the basics: what DNA is, how it is packaged inside sperm, and what happens when that DNA becomes fragmented. Lalana explains the difference between single-stranded and double-stranded DNA fragmentation, why different types of damage may have different implications, and some of the factors that can contribute to sperm DNA damage. We then explore how DNA fragmentation fits into the wider fertility journey.A standard semen analysis provides important information about sperm count, motility and morphology, but it does not assess sperm DNA integrity in the same way. So when might DNA fragmentation testing be considered? Why isn't it routinely offered to every man experiencing fertility problems? And how should men understand the results they receive? We also discuss recurrent pregnancy loss, IVF, ICSI, testicular sperm retrieval, fertility clinic practice and the current evidence around DNA fragmentation testing.Importantly, this isn't a conversation about presenting DNA fragmentation as a simple answer to infertility. Lalana explains where the evidence currently stands, what we still need to understand, and why further research is needed to establish how earlier testing might influence natural conception, fertility treatment outcomes, safety and cost effectiveness. We also finish by looking at male preconception health and sperm health, including lifestyle, diet, stress, sleep and other factors that may influence sperm DNA integrity.Questions we explore in this episodeWhat is sperm DNA fragmentation? What does DNA fragmentation actually mean? Can you have a normal semen analysis but still have sperm DNA fragmentation? What causes sperm DNA fragmentation? What is the difference between single- and double-stranded DNA fragmentation? How is sperm DNA fragmentation tested? When might DNA fragmentation testing be considered? Why isn't DNA fragmentation testing routine?Could sperm DNA fragmentation be relevant to recurrent pregnancy loss? What can men do to support better sperm health?About Lalana SongraLalana Songra is Chief Medical Advisor at Examen Lab. We're back. Briefly. This is our season finale, and before we sign off we wanted to sit down and actually tell you where we've been, what we've been up to, and what's coming. We've not been quiet. We've been at the Fertility Show, running panels, staffing a stand, talking to hundreds of people over two days. We've been building something in the community. And we've made a decision that's been a long time coming. We're moving on from the NeXYs name. The community is now the Male Fertility Hub. We sat in a meeting for an hour and a half going round the houses on what to call it, and landed back on the exact word we started with. Sometimes the obvious answer is the right one. It does what it says on the tin, and that's the point. We've also been listening. Men in the community kept asking where their partners could go for the same kind of support they were getting. We'd always assumed there was plenty out there for women, but what they meant was there was nothing specifically for the partners of men going through male factor infertility. So we built it. We now have a couples group, couples online meets, and a ladies-only group for female partners, run by Jenna and Jen. It's already making a difference. This is the last episode of the season, but there's a lot coming. We're looking forward to getting back to it.What to ExpectWhat made this year's Fertility Show the best one yet, and the message from a colleague that stuck with usThe name change: why we moved from NeXYs Fertility to the Male Fertility HubThe moment at a talk in March that finally settled the debateThe new couples group, couples meets, and ladies-only group for female partnersWhy we built it, who runs it, and how to joinWhat's coming in the next season and why we're more fired up than everFind us at themalefertilityhub.com. Choosing a supplement can be just as confusing, especially when many multivitamins contain strong antioxidants that can actually harm sperm quality.Impryl is different. It's made using activated micronutrients that support every cell in the body, from the largest, the egg, to the smallest, the sperm. These nutrients have been clinically shown to significantly improve live birth rates in men and women who had previously been struggling to conceive. What we really like is that behind the science are real stories from real people. To find out more, visit fertilityfamily.co.uk. You know you should be eating better, sleeping more, cutting back on alcohol, exercising differently, managing stress, and taking supplements. And knowing all of that is somehow making everything worse. In this episode, Ciaran and Shaun take on a brilliant listener question: how do you actually optimise your lifestyle without becoming obsessive about it? It is one of the most common traps men fall into during a fertility journey, and both hosts have lived it themselves.Shaun spent eight months following a strict low-GI diet to try to bring his testosterone levels up naturally after his micro-TESE, with charts inside his kitchen cupboard and every food on the glycaemic index memorised. His levels went down. Ciaran was handed a list of changes in a consultation room and immediately buried his head in the sand because the whole thing felt so overwhelming he didn't know where to start. This conversation is honest, practical and genuinely reassuring. If the pressure of getting everything right is starting to feel like another thing to fail at, this one is for you. What to ExpectWhy trying to do everything at once often makes things worse, not betterShaun's eight months of obsessive low-GI living and why his testosterone still went downWhat Ciaran's urologist said about alcohol that actually stuckWhy fertility nutritionist Mel Brown told her clients to enjoy the burgerCiaran's practical framework: write down five changes, prioritise them, and introduce them one at a timeHow to think about consistency rather than perfection, and why one bad week doesn't matterReframing lifestyle changes as something that outlasts your treatment, not a programme you are onThe thermostat analogy that Shaun still uses years laterCiaran is available for direct support and guidance via Instagram @themalefertilitypt or by email at [email protected] your questions for a future Male Bag episode through any of the channels below.
