Jennifer sent in a great question to "Ask the Egg Whisperer." She is 38, and from Seattle. Here's what she wrote," Hi Fertility Queen, you are the BEST! I'm 38 years old & diagnosed with diminished ovarian reserve. I've been through 5 rounds of IVF and have 2 5AA frozen embryos right now.  My husband and I want 2 kids therefore tried to bank another embryo but the 5th round failed therefore we are moving on to a FET.  I did a mock transfer and the BCL6 from the Receptiva came back positive therefore we are doing 2 Depot Lupron shots a month apart prior to the real transfer. My environment is good (healthy eating, lots of exercise, limited toxins in our beauty and cleaning products, great therapist). I'm taking a Prenatal with folate and DHA, Vitamin D, & Vitamin E.  Are there any other supplements I should take?  I've heard NAC may be good for endometriosis. Is there anything else I can do to prepare for this transfer?  I am willing to do whatever it takes to help my pregnancy chances.  Thank you so much!" The brief answer for Jennifer is this: 1. As part of the IVF cycle, patients are given hormones which stimulates the ovaries to allow all the possible follicles and eggs to grow. The good news is that this often allows us to capture as many healthy eggs as possible in one cycle, giving a patient a better chance at a healthy embryo. Dr. Geoffrey Sher is back, and we are going to be covering a very important topic: why do you need to do PGT testing if you’re doing IVF (or do you?) Dr. Sher is the co-founder of Sher Fertility Solutions (SFS) and internationally renowned expert in the field of ART (Assisted Reproductive Technology) and has been influential in the births of over 17,000 IVF babies.  Around 2005, Dr. Sher's team published the first paper that showed beyond a shadow of a doubt that if you were able to fully karyotype the embryo cells  that a euploid embryo would have an excellent chance. Today there is hardly an IVF program in the world that is not doing PGS/PGT-A to assess embryo viability. Today, we are discussing if PGT is needed for all IVF cycles, the role DOR plays in assessing if PGT is useful, and other maternal chromosomal and genetic abnormalities that may mean you'd want to consider PGT. Plus, he's addressing the concern that some patients have around the possibility that PGT may harm an embryo (short answer: if done correctly it does not). I’m honored to be joined by Dr. Carol Burton, who is a reproductive biologist practicing clinical embryology. Her active research interests span reproductive biology, from basic embryology to cutting edge artificial intelligence applications for IVF. She is the founder of ARTCompass, which is a free open source image based embryology survey tool, and she is the recent author of The Thin Pink Line: Regulating Reproduction. Her book is truly all about empowering people by educating them about present-day drivers of access, structural inequality, and quality of care issues in reproduction. Dr. Geoffrey Sher is joining me again, and this time we are going to talk about implantation failure during IVF. Dr. Sher is co-founder of Sher Fertility Solutions (SFS) and an internationally renowned expert in the field of assisted reproductive technology, and influential in the births of more than 17,000 IVF babies. One day I hope that I’ll be able to say the same thing about myself. Over the last 30 years, he has helped fashion the entire field of ART after training under the fathers of IVF, Dr. Patrick Steptoe and Robert Edwards. He established the first private IVF program in the United States in 1982, then later expanded his practice to include several centers throughout California before founding the first Sher Fertility Solutions in Las Vegas. He’s also the author of several books, In Vitro Fertilization: The ART of Making Babies, which is now in its third edition. In this episode, we're talking about implantation failure during IVF. Many times I wonder why embryos do not stick and grow. Dr. Sher is discussing the role of the endometrium, the difference between implantation and miscarriage, and the three factors he considers when advising on treatment for implantation failure. Meaghan Doyle is a genetic counsellor at Markham Fertility Centre in Ontario, Canada, specializing in infertility, preimplantation genetic testing, and preconception genetic counselling. After seeing patients who came from other clinics where access to a genetic counsellor was not available, she saw the negative impact this had on their care. She has since joined Instagram to help provide education on fertility genetics to help bridge this gap. Meaghan is passionate about genetics, and works together with patients so they can feel empowered to make decisions about their fertility treatment that are best for them.
