This episode gave me a terrible wonderful idea: full larp d&d game but everyone is playing homebrew subclasses that make rolling dice in combat canon.

I think in the restaurant case, the DM didn't "have to" give them something interesting, as they phrased it, but doing so is good dming, unless the fact it is utterly normal is itself important. After all, this is a fantasy world, nothing is really "normal and just like completely ordinary with nothing strange or magical or mischievous going on" UNLESS the strange or magical or mischievous thing is the fact that no such thing is happening (ie if the restaurant was sealed from strife by magic means or was actually a huge ass mimic in the habit of providing deceptive comforts). And if there's nothing strange about the place itself, then there is no reason not to have any random NPC use its normalcy as a cover for their own misdeeds.
