Planning for transition from medical locations is best handled BEFORE it’s needed. Lori and Kerri explain how care management happens when a fall happens and they need help while they heal. Sante has 24-hour coverage and multiple layers of assistance to assure safety is maintained and care is consistent giving all patients the best circumstances to heal. Kerri explained to be eligible for the benefit of rehabbing there, people need a daily skilled nursing need to attend. Tours are available anytime and often decisions are made last minute so to help folks feel ready, they encourage families to stop by and get acquainted with a tour.
