Description

A coroner's inquiry has revealed flawed Corrections processes played a role in the murder of Christchurch woman Nicole Tuxford. Coroner Marcus Elliott found that Corrections failed to identify the acute risk to Ms Tuxford's life at the time of her death because of a systemic issue with the department's processes.
