Tragic Zipline Accident: A Workplace Safety Managers Death in Far North QueenslandDean Anson Sanderson, a workplace safety manager from Adelaide, tragically died on a zipline tour in Cape Tribulation, far north Queensland, in October 2019. The tandem zipline tour with his wife, Shannon, ended in disaster when they lost momentum midway across an 86-meter section of wire and fell. Shannon suffered serious injuries, while Dean suffered fatal head and chest injuries.The incident was attributed to serious flaws in the zipline ride, with ropes attached to end points using bulldog-grip clamps that were not sufficiently tightened. A safety consultant found their torque strength was well below what was required, leading to the termination on the zipline completely failing on the day of the incident.The company operating the zipline, Keydane, stopped operating the day of the incident and is no longer registered. A former director was found not guilty of workplace safety failures, and a case against the company was discontinued. Mr. Sandersons death will be the subject of a five-day inquest beginning on March 2, 2026, with Mrs. Sanderson and other witnesses from the tour expected to give evidence.
