These are the exact consequences I give in my classroom when my students are out of their seats, blurting or talking out while I'm teaching, refusing to do work, etc. Years ago I used to use a clip chart and absolutely dreaded it. Now my students consistently follow expectations in class because of these consistent consequences they have in class.
