Having worked with a number of practices, a common problem we encounter is the lack of definition of the job roles and responsibilities of its team members. The danger here is that without having a clear direction of what the staff is meant to do on a daily, weekly, or monthly basis, it could lead to confusion, missed or forgotten tasks, or double handling.
