The coroner found that the Premier, state government ministers, and the Department of Justice repeatedly ignored warnings that a child would die inside the unit. Clevelands cell had no running water for twenty-one days before his death, and he was locked in his cell for more than twenty-two hours a day for seventy-four of his last eighty-seven days there. The coroner made fifteen adverse findings against the department and nineteen recommendations, including the immediate closure of Unit Eighteen. However, the state government has not supported this finding, nor a recommendation for a special inquiry into how Unit Eighteen was established. The facility was set up quickly after riots at another youth detention center.
