Feline diabetes mellitus (DM) presents unique challenges for practitioners and pet owners, often complicated by cost, time commitment, and concurrent disease. This episode explores practical strategies to make DM management more accessible and sustainable without compromising patient welfare. We review current diagnostic standards, including Project ALIVE’s consensus criteria, and discuss flexible treatment approaches beyond rigid protocols. Emphasis is placed on improving owner engagement through clear communication, staged treatment initiation, and individualized care plans. Monitoring options—such as fructosamine, blood glucose curves, urine glucose, and clinical scoring—are compared for their pros and cons, highlighting the importance of tailoring methods to each cat-owner pair. We also address remission: while possible, it should be considered a bonus rather than a primary goal, given variable success rates and relapse risk. Finally, we examine the impact of concurrent conditions and the need for balanced decision-making when adjusting other therapies.
