'Rapid Reviews' and 'Local Child Safeguarding Practice Reviews' have now replaced Serious Case Reviews in England. Dr Penny Sorensen and Professor Jonathan Dickens join us to talk about the first independent anual review of the new system for reviewing cases of child death and serious injury.
