Whoever wants to see the inside of this temple has to make a reservation. You have to fill in this form. You have to fill in the column boxed with a bold solid line. You have to write your name, address, and phone number. And you have to write the names of all the members who will come with you. Then in the bottom of the column, you have to check the time when you want to visit. After filling it in, send it at least two weeks before your visit. In a few days, you will receive a permission card with names of all the members on it.
