Barbie Look is NOT for everyone.”Exploring Labiaplasty Techniques: The Perfect Fit for Your Anatomy and DesiresWelcome to this special edition of my Feminine Wellness Newsletter, where I dive into the world of labiaplasty choices. I invented the Barbie Look and Hybrid Look Labiaplasty in Laguna Beach, California, in 2005, and have taught my methods globally, with emphasis on precision radiofrequency surgery for minimal scarring and optimal results. In this issue, we'll break down how to choose the right labiaplasty technique based on your unique anatomy (like the size, shape, and pigmentation of your labia minora and majora) and personal desires (whether you want a dramatic change, natural preservation, or something in between). We'll start with the Big Picture: Linear Excision versus Wedge Excision. Then, we'll zoom in on linear excision options, including my signature looks—Barbie, Hybrid, and Simple Trim—to help you decide what's best for you. Remember, this is educational content; always consult a Cosmetic Gynecology Specific Board-Certified Specialist for personalized advice. Step 1: Linear Excision vs. Wedge Excision—Which Base Technique Fits Your Needs? Labiaplasty primarily addresses the labia minora (inner lips), which can become enlarged due to genetics, hormones, childbirth, or aging, causing discomfort during activities like exercise, sex, or wearing tight clothing. The two main techniques are Linear Excision (also called Trim) and Wedge Excision. Both aim to reduce length, create symmetry, and tuck the labia minora within the labia majora (outer lips), but they differ in approach, outcomes, and ideal candidates. Linear Excision (Trim Technique): The Straightforward Edge Remover This method involves a curved or straight incision along the outer edge of the labia minora to trim away excess tissue, often following the natural contour for a "Curvilinear" result. It's like sculpting the edges to shorten and thin the labia while removing pigmented or thickened borders. I use radiofrequency energy in most cases for precise cutting and sealing, minimize bleeding, swelling, and scarring—key for my advanced variants. Pros:- Safer healing with lower risk of wound separation or Swiss Cheese Holes - Excellent for addressing pigmentation issues, as it removes the darker outer edges, resulting in uniform, lighter-colored labia. - Highly customizable for asymmetry—surgeons can adjust the amount removed on each side independently. So much more choice and options on how much to remove.- Versatile for dramatic or subtle changes, making it ideal for aesthetic goals like a "petite" or streamlined look. Cons: - The straight or curved edge may look less "natural" if not done artistically and not following the natural curves of the labia potentially creating a more uniform but less contoured appearance. It can look “Too Plastic.” - Scalloped edges if sutured too tightly- In most cases, Linear Excision will require a companion clitoral hood reduction if the hood appears prominent post-surgery. You have to avoid the “Top Heavy” look that can occur of you leave the top clitoral hood alone and then dramatically trim down the middle and bottom labia minora. Who Should Get Linear Excision?Opt for this if your anatomy includes uneven pigmentation, thickened edges, or significant asymmetry that needs targeted removal. It's best for patients desiring a cleaner, more uniform aesthetic—think women who want to eliminate dark borders or achieve a "hidden" look without preserving the original edge texture. Functionally, it's great for those with irritation from rubbing or pulling, especially if you prioritize quicker recovery and minimal complications. Athletes or active individuals often favor it for its reliability in high-movement lifestyles. Swimmers, bikers, horseback riders, volleyball and tennis players are examples. If your labia majora are fuller, linear excision ensures the minora stay tucked away seamlessly. The chance of having Swiss Cheese Holes is less than 1%. Wedge Excision: The Natural Contour Preserver This technique removes a V- or wedge-shaped piece of tissue from the widest part of the labia minora, then stitches the edges together to shorten without altering the outer border. It can be more complex and involving advanced modifications like Z-plasty for better blood flow and reduced tension. Variants include Central Wedge Excision, Superior and Inferior Wedge Excision. Pros: - Preserves the natural ruffled edge, color, and contour of the labia, leading to a more "original" appearance that blends seamlessly with the clitoral hood. In my personal practice and in clinical research done by highly respected friends of mine, only a very small percent of women actually want to keep that ruffled and dark edge. Only about 4-6% of women wanting labiaplasty want to keep the original edges. About 94-96% of them want those darker edges removed and made smoother. This is the perfect choice for women who want to keep that original edge appearance.- Scarring can be less visible once healed in many but some may continue to have a pale white line at the site of suturing. - Effective for reducing length without changing the labia's overall texture, can be ideal for subtle functional improvements. Most wanting labiaplasty are not looking for a subtle change and seek more dramatic changes. Cons:- Higher Tension on the closure site pulling the wedge edges apart, with a significant risk of dehiscence (wound reopening), Pac Man Breakdown, or Swiss Cheese Holes. In the very best of hands this wound breakdown rate averages 10-20%. Compare that to less than 1% Swiss Cheese holes and less than 5% dehiscence when the Linear Labiaplasty is done. - Doesn't address pigmentation well—if your edges are dark or irregular, they remain. You keep those dark and ruffled or irregular edges. - Can be more technically demanding, so outcomes vary by surgeon experience; poor execution can lead to bunching or narrowing or dehiscence and Swiss Cheese Holes. The dreaded Pack Man Look happens 10-20% of the time.Who Should Get Wedge Excision?This is ideal if your anatomy features a naturally pretty edge you want to keep, with hypertrophy mainly in the inner width rather than the borders. Choose it if your desires lean toward maximal preservation of your innate look—women who value a "seamless, untouched" aesthetic or have minimal pigmentation concerns. It's particularly suited for those with pain from labial hypertrophy but who prioritize natural contours over color uniformity. Avoid it if you have very redundant tissue, as it may not remove enough for dramatic results. Key Decision Factors: Anatomy (e.g., edge quality, majora fullness) and desires (dramatic vs. natural) guide the choice. A 2021 survey showed curvilinear (trim variant) as the most common (52.7%), followed by wedge (36.1%), reflecting its versatility. In speaking with the most renown and exceptional labiaplasty surgeons around the world (primarily Gynecologists) it is closer to 90% Curvilinear Labiaplasty versus Wedge Labiaplasty. I prefer linear/curvilinear techniques for fewer complications, and I avoid wedge techniques altogether. I just do not want problems. Step 2: Within Linear Excision—My Alinsod Looks: Barbie, Hybrid, or Simple Trim? If linear excision aligns with your needs, my radiofrequency-enhanced curvilinear approach offers refined sub-techniques. These vary by how much tissue is removed, balancing aesthetics, function, and reversibility. All use precise energy to cut and cauterize, reducing downtime and results in speedy recoveries and fewer complications.The Barbie Look: The Ultimate Tucked-Away Transformation I invented this in 2005. This aggressive linear excision removes *all or nearly all* of the labia minora, creating a smooth and straight vertical line with no protruding inner lips, even when legs are apart. It often pairs with clitoral hood reduction for a fully streamlined vulva. Where did this “Barbie Look” terminology come from? It was from my patient in the San Fernando Valley who came up to me and announced “Dr. Alinsod, I want to look like Barbie!” I said to her “Barbie has no labia!” She said “EXACTLY!!!” The Barbie Look was born. Don’t blame me for the lay public terminology because it came from my lay public patient who visualized a certain look and gave it a certain name. And it went around the world like a wildfire. I did lots of interviews for magazines and got invited around the world for me to show what in the world this Barbie and Hybrid stuff is all about. Thank you San Fernando Valley!Who Should Get the Barbie Look?Anatomy: Best for those with sufficient labia majora volume to fully cover the vaginal opening post-removal—flat or deflated majora might leave you feeling exposed. Ideal if your minora are extremely elongated (e.g., >4-5 cm) or asymmetrical beyond correction and you have lab

Navigating Genitourinary Syndrome of Menopause (GSM) – Reclaiming Comfort and IntimacyAs women enter menopause, bodies undergo profound changes, many of which are rarely discussed openly. One such challenge is Genitourinary Syndrome of Menopause (GSM), a condition driven by falling estrogen levels that affects the vulva and vagina. This can lead to vaginal atrophy, dryness, and tightness, making everyday activities—and especially intimacy—painful or even impossible. Today, I am shedding light on this often-overlooked issue, its impacts, and the empowering steps you can take to restore your vaginal health. Remember, you're not alone, and there are effective solutions available. I specialize in this problem. I transform the saggy to tight and the overnight to comfortable. What is GSM and Why Does It Happen?GSM encompasses a range of symptoms resulting from estrogen loss during menopause. Estrogen keeps vaginal tissues elastic, lubricated, and resilient. Without it, the vaginal walls thin, lose moisture, and become less flexible, often leading to a "too tight" vagina. This isn't about muscle strength but rather tissue changes: the vulva and vaginal entrance can scar, atrophy, or tighten due to aging, inflammation and reduced blood flow as estrogens plummet. Physically, this manifests as burning, itching, urinary issues like frequent infections or incontinence, and severe pain during penetration—known as dyspareunia.The emotional toll is equally debilitating. When sex becomes painful or impossible, it can strain relationships, erode self-confidence, and lead to feelings of isolation or inadequacy. Many women report anxiety, depression, or a loss of intimacy with partners, turning what should be a joyful connection into a source of dread. It's heartbreaking, but the good news is that GSM is treatable, and addressing it can rebuild both physical comfort and emotional well-being.First-Line Solutions: Restoring Your Vagina Non-SurgicallyThe goal of treatment is to revive the vaginal environment, increasing moisture, elasticity, and size for comfortable penetration. Start with conservative approaches, often recommended by gynecologists or urogynecologists. These can be highly effective and are typically tried before considering surgery:**Vaginal Estrogen Creams or Suppositories**: Low-dose topical estrogens (like estradiol creams) replenish hormones directly where needed, thickening tissues, boosting lubrication, and reducing tightness. Applied 2-3 times a week, they often show improvement within weeks. Always consult your doctor, as they're safe for most but may not suit everyone (e.g., those with certain cancers).**Dilators**: These graduated, smooth devices help gently stretch the vaginal opening and walls. Start small and progress, using them with lubricant for 10-15 minutes daily. They're empowering tools for self-care and can prevent further tightening. In most cases the combination of vaginal estrogens and manual/dilator manipulations are all you need to get back to normal. **Pelvic Floor Physical Therapy**: A specialized therapist uses techniques like biofeedback, manual massage, and exercises to relax tight muscles and improve tissue flexibility. This addresses not just GSM but related issues like pelvic pain.**Carboxytherapy**: This involves injecting medical-grade CO2 gas to stimulate blood flow and collagen production, rejuvenating atrophied tissues without downtime. This is available in Latina America and Europe but not in the USA. Carboxytherapy Gel made by Lumisque and Called CO2LiftV is an excellent choice and available in specialized doctor’s offices. I helped bring this therapy into the USA.**Plasmaporation, Radiofrequency (RF) and Laser Therapies**: Energy-based devices like Plasmaporation, ThermiVa (RF), or CO2 lasers heat tissues to promote regeneration, increasing collagen and elasticity. Sessions are quick, non-invasive, and can dramatically reduce dryness and tightness. Multiple treatments (3-5) are often needed for optimal results. I invented and developed the use of radiofrequency to aid the aging vagina and vulva.These options are backed by research and can return many women to a "normal" state of comfort and function. Patience is key—results build over time.When Conservative Treatments Aren't Enough: The Surgical OptionIf you've exhausted non-surgical methods without relief, surgery may be the next step. One innovative procedure is the "Band Release," designed to address persistent tightness from scars or thinned but rigid tissues. This minimally invasive office procedure involves precise incisions to release constricting bands at the vaginal entrance, effectively enlarging the diameter for easier penetration and pain-free sex. Performed under local or general anesthesia, it typically takes less than an hour with a recovery period of 6-8 weeks, during which you'll avoid intercourse as you heal but can do most all normal things in life like walk, work, workout, drive. My surgical focus has developed techniques to reverse this problem, including advancements in the Band Release and related aesthetic vaginal surgeries. As a focused urogynecologist and vulvovaginal restoration, I have specialized in restoring vaginal health for women affected by GSM, atrophy, and related issues, often combining surgical precision with non-invasive technologies like radiofrequency. Here is my Reader’s Digest short version of www.alinsodinstitute.com’s coverage of the basic problems women face when aging and gravity starts to win. Click on the section titles to lead you to more content.Red Alinsod, MDIncontinence SlingsThe “Slings” you hear about are for Stress Incontinence (leakage of urine that occur with laughing, coughing, sneezing, jumping, trampolines. Slings are made of polypropylene, an inert nylon, that is placed right under the mid urethra to act as a backboard when pressure from above (cough/sneeze/laugh/jump) compresses the urethral tube into the sling to prevent leakage. Slings are “Tension-Free” because the slings are free floating and not sutured into any structure. This is the current Gold Standard of incontinence surgery.You may hear the term “TVT” or “TOT” which refers to the route the slings are placed. TVT, or tension-free vaginal tape is placed through incisions right above your pubic bone. TOT (transobturator tape) is placed through incisions on the crease of your inner thighs. Once healed you can barely see these incisions. Both procedures are outpatient surgeries of about 15 to 30minutes and can be done with pelvic floor repair and cosmetic procedures such as labiaplasty and vaginoplasty. The success rates vary from 80 to 95 percent. Occasional complications of mesh eroding through skin, urethra, or bladder do occur. Other complications are slings that are too tight preventing urination. I use a sling system I helped design back in 2002 which has proven to be safe, works well, and is enduring. They are shown in the example surgery below with my hands holding them.The Luksenburg System for Mesh-Free Incontinence TreatmentAriel Luksenburg, M.D., a gynecologist from Uruguay, introduced his system for the treatment of severe stress incontinence to the U.S. in 2021 at the International Society for Cosmetogynecology Pre-Congress workshop in Fort Lauderdale, FL. The course was repeated in 2022 and most recently in March of 2023. This time with FDA clearance to use his threads and patented delivery system. After hearing the system patent was granted, the PCL threads becoming CE and FDA compliant, and seeing the 3-year data presented, I became a believer. I was honored when Ariel Luksenburg and Jorge Gaviria invited me to become the first surgeon in the U.S. to be trained, perform, and to teach The Luksenburg System. They committed to also train my California colleague Judy Wei, MD. We trained on July 24 and 25, 2023, in Irvine, California, at Dr. Judy Wei’s beautiful office and now offer this in Dallas, Las Vegas and Irvine.Here is a link to my Feminine Wellness Newsletter Video of the procedure: https://open.substack.com/pub/femininewellness/p/the-luksenburg-system-for-the-leaky?r=8vc2b&utm_campaign=post&utm_medium=webCystocele Repair or Fallen Bladder RepairA bulge of skin that can be felt or seen coming out of the vagina looking like an egg or a golf ball is usually the bladder or rectum. Sometimes it is the urethra, cervix, uterus, or top of the vagina if there has been a hysterectomy. Urinary leakage often accompanies this bulge if it is the bladder falling down along with the tube that drains it called the urethra. Us gynecologists call this “Prolapse.” Pelvic pressure, feeling of fullness and heaviness are common symptoms. This is mostly caused by childbirth trauma and the aging process with loss of collagen support. The average age of my repair patients is 54. Sometimes there are minimal to no symptoms and you just see an unsightly mass. Cystocele repair (also called Anterior Repair or Anterior Colporrhaphy) is the surgical elimination of the bulge and restoring normal anatomy. This is done in the Surgery Center or hospital under general or spinal/epidural anesthesia. Traditional techniques of bunching or overlapping tissues with suture has a very high failure rate ranging from 25 to 60 percent. It is a challenging surgery. Modern repair treats this bulge as a hernia of the bladder going into the vagina aided by a graft to build a new firm wall. I have performed and taught this augmented surgery in the United States since 1997 with success rates of about 90% when using a dermal patch (cadaver skin). I currently use the product made by ARMS Medical called VNew Dermal Allograft. I designed the specialized shapes for its bladder repair and rectal repair products. The use of nylon patches works very well with failure rates less than 5% but the FDA has removed most of the nylon mesh products in the market. A company called Coloplast has an ultra-lightweight mesh that experienced Urogynecologist have used safely and successfully. I brought this into the United States from Scotland in 2004. It was first called POP Mesh, then MPathy Mesh, and now Restorelle. I may use this product for the most severe cases or use it for prior failed surgeries.Rectocele Repair or Fallen Rectum RepairWhen the bulge into the vagina comes from the rectum it is called a rectocele. As with other forms of pelvic organ prolapse (cystoceles, enteroceles, vaginal prolapse) childbirth, chronic cough, chronic constipation, and obesity are predisposing factors. Symptoms are similar to cystoceles such as pelvic pressure, an unsightly bulge in the vagina, and constipation. It is sometimes hard to tell if the bulge is from the bladder or rectum and the examining physician will need to use a “Half Speculum” to define what is hanging out. Furthermore, the need of reaching into the vagina to push stool out is not uncommon and a giveaway that the rectum is falling out and giving symptoms. Of course, both the rectum and bladder can fall out together at the same time. Surgical repair consists of using sutures to bunch up the bulging tissues together or bringing muscles together into a new flat floorboard. More modern repair consists of the use of mesh or donor tissues. Rectocele repairs have a 90-95% success rate when the muscles are approximated together.Uterine SuspensionWhen a hard lump is felt or seen hanging out the vagina it’s usually a cervix and uterus and often with a bladder attached. A hysterectomy and suspension of the top of the vagina is the traditional cure. It works very well. If you don’t want a hysterectomy, then I can suspend the entire structure falling out with a suspension procedure I invented almost 20 years ago. This is a great option for those with a small cervix and a small uterus typical of menopausal women. I have taught this technique worldwide. I use either cadaver tissue or ultralightweight mesh as suspensory and supporting pseudo-ligaments like a pulley system. I do these in a surgery center or operating room under general anesthesia. It takes me 60-90 minutes. Again, a high-volume surgeon for this specific procedure is important to find if you want the best outcomes.Vaginal Vault Prolapse (Fallen Vagina)After a hysterectomy a vagina that loses its support may come down and out like an inside-out sock. If a woman still has her uterus then this is called a uterovaginal vault prolapse. If only the uterus falls out and the top of the vagina is still well suspended then it is called a uterine prolapse. Vaginal vault suspension can be done in many ways. You can use the patient’s own weakened tissues or augment the repair using a biologic product such as human cadaver skin dermis or polypropylene nylon. Some physicians prefer an abdominal approach to attach the top of the fallen vagina to the sacrum. Some highly skilled surgeons do this laparoscopically with a Sacralcolpopexy. That is when you suture the top of the vagina to a bony prominence of the sacrum deep inside the pelvis.Robotic surgery has made this easier this past decade and today’s recently trained graduates use this route as their primary approach. The vaginal approach to suspending the top of the vagina used to be the most common but it is becoming less and less as their surgical numbers in training programs decline. The top of the vagina can be sutured to the uterosacral ligaments or to the sacrospinous ligaments through the vaginal approach. Either approach works well with different complications to consider. The vaginal surgery skills needed for this excellent technique is getting lost in today’s academic training programs. You have to find a high-volume surgeon that gets enough of these cases to remain competent. Red Alinsod, MDAlinsod Institute for Aesthetic Vulvovaginal SurgeryDallas, TXBACKGROUND on LABIAPLASTY:In 2005 I developed the Barbie Look Labiaplasty, a radical and aggressive form of labiaplasty that removed almost all to all of the labia minora to achieve maximal comfort for athletes, reduce the outlining of the labia when wearing swimsuits and leotard, and as a cosmetic preference to obtain a “clamshell” look wanted by women in Southern California. This was an athletic, smooth, and sleek look that was the most “tucked in” you could get. It was impossible to achieve this look with the standard plastic surgery wedge labiaplasties. It has been a beloved surgery for the carefully selected patients who want this minimalist “Look” it. No more adjusting and tugging. My Olympic level athletes found it to be an outstanding choice.HISTORY:Soon after my Barbie Look Labiaplasty technique became well known through my teaching programs, the media, presentations in Congresses, it was copied worldwide and attempted by untrained surgeons. Poor and awful outcomes were rapidly occurring, and I was referred many patients for repair. Patients came to me with no labia minora at all when they did not seek out or ask for the Barbie Look Labiaplasty. Their surgeon chose the wrong procedure of a Barbie Look for patients who wanted labia left. Many surgeons who had done zero to only a few labiaplasties ended up not knowing of the massive retraction of tissues in the upper third of the labia minora with resultant retraction of labial edges to end up flat and with NO labia at all. These surgeons did not know what to do and I was often consulted. I was also sought out as an expert witness for malpractice cases. I had a small handful of patients in the first decade of my practice doing the Barbie Look Labiaplasty who changed their minds months to years later and wanted to have a little bit of labia minora showing and not be completely flat. The pictures below is one of those patients. My original solution was simple and worked well if they wanted just a little bit of labia minora below the level of the labia majora. If they wanted more of the labia above the level of the labia majora I would have to make labia majora incisions up to 2cm above where the labial crease would have been. I called this my Grooving Technique because I created a groove between where the labia minora and labia majora had met, the interlabial fold, undermined medialy, and created a labia from no labia. It was actually creating an optical illusion of a new labia when in reality it was creating a groove and sometimes suturing the lateral majora edge to the lowest portion of minora edge that was just “grooved.”I shared this secret with Amr Seifeldin when he visited my office on October 6, 2015, 2017, 2019. I did not publish my technique and only shared it with my Alinsod Fellows who would travel to Laguna Beach over the past 18 years to participate in my Preceptorships. Amr expanded on my methods for the patient who wanted a more significant labia minora reconstruction without the use of flaps and has taught this to his Fellows. His Center in Cairo, Egypt, has been a tremendous resource for patients in the area wanting both Female Genital Mutilation Repair, Cosmetic Gynecology Surgeries, and advanced Revision Labial Surgeries such as my Grooving Technique. For the patient who wants to create the illusion of a labia minora where there was none up to the level of the labia majora and just past it.2. Local anesthesia3. Use radiosurgery to create a “Groove” between the labia minora and labia majora, where the crease used to be, the former intralabial fold.4. Start with the fine pinpoint “Hair Tip” of the Ellman or Soniquence RF to outline and incise from the top of the labia minora to the introitis.5. Use this “Hair Tip” to smoothen any labial and hood areas to create a smooth from from the clitoral hood to the frenulum or its remnants, to the labia minora.6. Use the small ball tip to continue the Grooving deeper while maintaining hemostasis to create a flab of labia minora medially. With adequate numbing cream, a QTip is used twice daily to keep the edges from fusing together. This is performed for 4-6 weeks. No suturing needed.7. For more significant labial creation, suturing is need to attach the lateral majora edge of the incision to the lowest portion of the medial minora edge to keep the appearance of a new labia minora.8.
