#THIRDTHURSDAY: Overview of Headache Evaluation and Treatment for Physical Therapists (June 2024)

June is recognized as National Migraine and Headache Awareness Month. Headaches are among the most common disorders of the human nervous system and can often wreak havoc on a sufferer’s daily life. Headaches can cause pain in the neck, sinuses, face, and head and can last for hours or days. This #THIRDTHURSDAY lecture will review etiology and classification of headaches disorders - focusing on how to obtain a neurologic history and perform a headache focused exam. Headache 'red flags' (worrisome complaints) will be discussed along with some basics of headache treatment along with physical therapy modalities that may help headache patients. Dr. Smriti (Simi) Wagle, D.O. shares insights and expertise on "Overview of Headache Evaluation and Treatment for Physical Therapists" What to expect:  Upon completion of this course, participants will be able to: Describe common headache types and understand how to perform a headache focused exam Have knowledge of 'red flags' in a headache history Demonstrate an understanding of basic physical therapy techniques that may help patients with headaches About the Speaker:  Dr. Smriti (Simi) Wagle, D.O. is a practicing general neurologist at Lutheran General Hospital in Park Ridge, Illinois. She completed her training in 2003 and sees patients in the outpatient and inpatient settings, reads EEGs and also performs EMG/nerve conduction studies. She is originally from Iowa and did her undergraduate and medical school training there. She completed her neurology residency through the Michigan State Health System and then an EMG/neurophysiology fellowship through the Mayo Clinic Health System. Since completing her training, she has chosen to practice general neurology and cares for patients with all type of neurologic disorders including: seizures, strokes, Parkinson's, memory loss, neuropathy and of course, headaches. She is an associate professor and the assistant clerkship director for Neurology at the Rosalind Franklin University Medical School. #THIRDTHURSDAY: PTs Who Order Diagnostic Imaging (May 2024)

In recent years greater attention has been given by our profession to the role of diagnostic imaging in physical therapy (PT) practice. Educational content related to diagnostic imaging is now required for PT program accreditation. Military PTs and all other civilian first contact providers routinely utilize diagnostic imaging in this critical decision-making process. As more therapists continue to step into direct access roles, it may be beneficial to evaluate how referral for diagnostic imaging may impact clinical decision making and ultimately enhance patient care. According to Illinois law, PTs are required to determine what conditions fall outside our scope of practice and to make referrals to appropriate healthcare providers when necessary. At times, a radiologist will be the most appropriate provider to refer to. This #THIRDTHURSDAY session will explore how the referral for diagnostic imaging studies by PTs can impact clinical decision making and ultimately lead to enhanced patient care. Multiple cases will be reviewed in which imaging results directly led to an alternative course of management. Sexual Harassment Prevention in the Workplace (1 CE)

Maintaining a safe and healthy work environment is a vital role for all employees and employers. This course will review sexual harassment in the workplace including various forms and manifestations of sexual harassment. IPTA's Ethics Committee will review steps to take if one experiences or witnesses sexual harassment in the workplace including how to report harassment to workplace supervisors, anonymous call in lines, and state agencies. Whistleblower protections will be reviewed for employees that report sexual harassment in the workplace. Case examples will be provided to apply course concepts on recognizing and reporting harassment. This course meets the 1 hour Sexual Harassment Prevention training required by the State of Illinois. Lauren is a Certified Lymphedema Therapist and has a professional background and interest in endurance. She is a Orthopedic Pediatric Physical Therapist with training in Video Gait Analysis and is Graston Certified. Lauren currently lives in Andersonville on the north side of Chicago. Ann W. Compliance with therapeutic exercise programs is generally poor, with over 70% of (low back) patients failing to perform their prescribed exercises. One of the many reasons for this poor compliance may be the frequent use of an internal focus exercise approach, which is often experienced as boring, repetitive, pointless, and non-productive. This course will challenge current belief systems about exercise and the way you practice and prescribe therapeutic exercise. This brief session integrates contemporary research, including the OPTIMAL Theory of Motor Learning, exergaming technology, and pain science into a new therapeutic conditioning and exercise method using primarily an external-focus approach. Compared to internal focus exercise programs, directed at the performer’s own body movements, external focus programs, directed at the effects of movement on the environment, have immediate beneficial effects on performance, retention, and transfer. Improvements are observed in movement effectiveness and efficiency, self-efficacy, confidence, automaticity of control, and overall cognitive function. Spotlight on Research: What’s New in the Dry Needling Literature

Every year, over 200 new research studies, systematic reviews and meta-analyses, literature reviews, and case reports are published in the scientific literature on myofascial pain and dry needling. For the clinician, these many papers make it nearly impossible to keep up with the literature.  In this brief review, Jan Dommerholt will explore the most pertinent recent literature on myofascial pain and dry needling. We will then progress to discussing the function of the rotator cuff and the potential pathophysiology of rotator cuff tears. #THIRDTHURSDAY: Understanding Arthrofibrosis and Its Clinical Implications (May 2023)

Arthrofibrosis, also known as joint contracture, is a pathological wound healing disease characterized by chronic inflammation and fibrosis of connective tissue in and around a joint, resulting in severe pain, marked loss of joint mobility, and diminished functional independence in daily life. It is most prevalent in the hip, elbow, and knee following exposure to physical trauma such as surgery, prolonged period of immobilization, or infection. In the shoulder, it typically occurs idiopathically and is known as adhesive capsulitis. In some cases, the disease may become permanent and, in rare instances, can result in amputation of the affected limb. Physical therapists are often the first members of the healthcare team to encounter an individual struggling with arthrofibrosis. Thus, early identification of the disease and proper clinical management are integral in preventing and/or minimizing its potential deleterious effects on an individual’s daily function and overall quality of life. This #THIRDTHURSDAY lecture will be a joint presentation, given by a physical therapist experienced in treating individuals with arthrofibrosis alongside a leading scientific researcher in the field. Arthrofibrosis and its clinical implications for the treating clinician will be discussed in detail and include a specific emphasis on arthrofibrosis of the knee following surgical intervention (e.g., s/p ACLR, TKA, meniscectomy). A current scientific understanding of arthrofibrosis and its pathological mechanisms will be presented first, followed by the presentation of common clinical markers for its early recognition in the clinic. Proposed best physical therapy practices when treating individuals struggling with arthrofibrosis will also be presented accompanied by relevant case studies. The presentation will conclude with a Q&A session amongst the presenters and lecture attendees. Dr. Nathan Brimm, PT, DPT & Dr. Kayley Usher, PhD shares their insight and expertise on "Understanding Arthrofibrosis and Its Clinical Implications." Nathan received his undergraduate degree in Kinesiology from the University of Illinois at Chicago and a Doctorate in Physical Therapy from Northwestern University. Over the past several years, Nathan has come in contact with countless individuals struggling with arthrofibrosis, especially following orthopedic surgeries such as ACL reconstruction and revision total knee arthroplasty. It is from these experiences that Nathan has sought to learn more about arthrofibrosis and its harmful effects in order to best optimize long-term clinical outcome of individuals suffering from this unyielding disease.   Dr. Kayley Usher, PhD received her PhD from the University of Western Australia and is an Adjunct Lecturer in the School of Biomedical Sciences at the University of Western Australia. Kayley has a research background in immunology and microbiology, and her journey with arthrofibrosis began after a total knee replacement. Her experience of the “black hole” in awareness and understanding of arthrofibrosis created a passion to inform and educate people. Drawing on her scientific understanding of fibrosis, Kayley wrote a review paper called "Pathological mechanisms and therapeutic outlooks for arthrofibrosis", published in Nature Bone Research in 2019. Since this time, she founded the International Arthrofibrosis Association where she provides scientifically referenced information about the pathology of arthrofibrosis and treatment options and provides support.   This program was originally recorded May 18, 2023. Alzheimer’s and Dementia: What Are We Forgetting? (1 CE hour)

Today, more than five million Americans are living with Alzheimer's disease, and 220,000 of these live in the state of Illinois. As the Silver Tsunami continues to roar towards our shores, the need for comprehensive and evidence-informed clinical management for our patients with dementia is more vital than ever. Increasingly, individuals with cognitive impairments caused by dementia are coming into contact with a variety of professionals across all healthcare settings. Due to the underpreparedness of our healthcare system in managing the complex needs of this patient population, the state of Illinois is requiring mandated training for all licensed healthcare professionals who see patients aged 26 or older. One hour of CE on dementia training is required each licensure renewal period effective January 1, 2023. This presentation serves as an introduction for physical therapy professionals on pathology, effective communication strategies, basic clinical care planning on dementia and the scope of practice of the physical therapy professional. This course meets the 1 hour Alzheimer and Dementias training required by the State of Illinois. The Dementia Training Mandate for Illinois healthcare professionals was signed into Law in the summer of 2021. For license or registration renewals occurring on or after January 1, 2023, all licensed healthcare professionals (as defined in Public Act 102-0399) in Illinois, who have direct patient interaction with adults age 26 and above will be required to complete at least a one-hour course in training on the diagnosis, treatment, and care of individuals with Alzheimer's disease and other dementias per renewal period. She completed her t-DPT in 2016 from A.T. Still University. Mitchelle has been an APTA, IPTA, and Geriatric Section member since 2010.  She is presently the Secretary of the IPTA Geriatrics Special Interest Group.  She lives in Du Quoin, Il with her husband, 12 y/o daughter, and 8 y/o son. Sexual Harassment in the Workplace: How to Protect Yourself and Your License (1 CE)

The Workplace Transparency Act (Public Act 101-0221), requires anyone who is employed in Illinois complete sexual harassment prevention training annually. For individuals with a professional license issued by the IDPR, with a CE requirement, must complete a 1 CE hour of Sexual Harassment Prevention Training, per renewal cycle. Maintaining a safe and healthy work environment is a vital role for all employees and employers.  This course will review sexual harassment in the workplace including various forms and manifestations of sexual harassment.  IPTA's Ethics Committee will review steps to take if one experiences or witnesses sexual harassment in the workplace including how to report harassment to workplace supervisors, anonymous call in lines, and state agencies.  Whistleblower protections will be reviewed for employees that report sexual harassment in the workplace.  Case examples will be provided to apply course concepts on recognizing and reporting harassment.  This course meets the 1 hour Sexual Harassment Prevention training required by the State of Illinois. COURSE OBJECTIVES:  Following completion of the course, participants will be able to: Describe sexual harassment. Recognize different forms and manifestations of sexual harassment that constitute unlawful action. Demonstrate an understanding of what one should do if they experience or witness sexual harassment. Demonstrate an awareness of state and federal statutes concerning sexual harassment including remedies available to victims of sexual harassment. Demetra John PT, PhD received her physical therapy training from Northwestern University and her PhD in Public Health from the University of Illinois at Chicago (UIC). She is retired after working more than 30 years at UIC in multiple faculty and administrative roles. Demetra is a member of the IPTA Ethics Committee and the IPTF Pro Bono Committee. She has provided pro bono physical therapy services to the uninsured for over 25 years. Paula Eppenstein, PT, MS received her physical therapy training from Northwestern University and her Master of Science degree from Northwestern University. Over the span of her career, she has worked in various levels of care and in numerous roles, including front line care, education and training and eventually leadership positions. She is semi-retired after working more than 35 years. She still continues to speak on healthcare related topics and is actively involved in simulation based training for medical and students in other healthcare fields. As of January 1, 2023, Illinois licensed physical therapists may certify that a patient has a medical condition that meets the requirements for the state to issue a temporary or permanent disability placard.  IPTA Executive Director, Colleen Flannery will walk through the forms, process, do and don'ts for certifying disability placards in Illinois. Implicit Bias: Recognizing and Responding in Healthcare Settings (1 CE hour)

Implicit bias is a form of bias that occurs automatically and unintentionally, that nevertheless affects judgments, decisions, and behaviors.  This educational session will introduce participants to implicit bias, where they come from, how they are formed, and explain how it differs from explicit bias. Participants will understand the impact of implicit bias on healthcare delivery and the harm it may cause their patients and clients. This session will utilize methods to help participants recognize bias and learn how to interrupt and mitigate the effects in their clinical spaces. This course meets the 1 hour Implicit Bias training required by the State of Illinois. Since this is a virtual live webinar, this counts as "live" CE. The Implicit Bias Training Mandate for Health Care Workers was signed into Law in the summer of 2021.  For license or registration renewals occurring on or after January 1, 2023, all health care professionals who hold a professional license (listed in Section 1130.500 Implicit Bias Awareness Training) shall complete a one-hour course in implicit bias awareness training per renewal period.  A licensee may count this one hour for completion of this course towards meeting the minimum credit hours required for continuing education.   What to expect:  Objectives:  By the end of this lecture participants will be able to: Explain the difference between explicit and implicit biases Describe causes of implicit bias; how they form and operate  Describe effects of implicit bias and the harms caused Recognize, interrupt and mitigate implicit bias. She is a certified clinical specialist in orthopedic physical therapy and an APTA Credentialed Clinical Instructor. She also serves in the role of Associate Chair of Diversity, Equity, and Inclusion in the Department of Physical Therapy.
