Many patients have drug therapy problems AFTER they start therapy, not before. The therapy isn’t working, it is causing side effects, or the patient isn’t taking it as prescribed. Or…everything is great. Assessing existing therapy is one of the most important roles of pharmacists, whether it’s renewals, med reviews, chronic disease management or checking in on a refill. The podcast series, 2 Cs 2 As (Control (Efficacy), Complications, Adherence, Adverse Effects), will look at ways to assess various treatments and conditions to support pharmacists in their care for patients and documentation.

In this episode, PANS' Glenn Rodrigues and 4th-year Dalhousie Pharmacy Student Jessica Condon-Oldreive catches up with returning guest Dan Rainkie to explore the pharmacist's role in managing congestive heart failure. Together they discuss key considerations in optimizing heart failure treatment, including medication selection, monitoring for adverse effects and supporting patient adherence.
