Comparing the Benefits of Fat Grafting vs Implants
When people discuss plastic surgery, it s usually within the context of specific, individual techniques. While this is a great way to have a conversation, it isn t a great way to do surgery. I refer to my approach as body contouring, because I believe in combining techniques and using each to accentuate the others.
Let s discuss Brazilian butt lifts, for example. I do quite a bit of work in Brazil and have operated in Rio de Janeiro, in Sao Paulo, and other parts of the country. My personal and professional relationships with many of the surgeons in that area allowed me to be an early adopter of this technique when it was first invented. The stereotypically desired female buttocks are high, rounded, and curved. A surgeon named Louis Toledo, a contemporary of mine, invented the Brazilian butt lift to achieve this look without, in most cases, a big operation or a significant amount of cutting.
The process involves adding fat to the upper aspects of the patient s gluteal area, so the entire buttock lifts up, tightening and presenting a more well-rounded curve. A side benefit of this is that, by raising the glutes, we are able to reduce the sag on the sides below the hipbone, which are often called saddlebags.
The fat used in this procedure is gleaned from other areas of the body where the patient might want to see some reduction: the waist, front of the thighs, abdomen, and maybe even the back and chest. This is called fat grafting. I pull from wherever the patient has excess fat and, after carefully processing it, inject it into areas we want to emphasize, such as the butt or breasts. I use this approach, rather than just a disjointed set of individual procedures, to holistically sculpt the body the patient wants.
Fat Grafting and Implants
The procedure of fat grafting is relatively recent and has been in common use for only ten years or so. The process has evolved rapidly, and we ve learned many valuable lessons. For example, we now know that if we inject 30 ounces of fat, only 20 will stay the rest will get reabsorbed into the body.
Although the core process of the Brazilian butt lift involves simply injecting fat, sometimes there isn t enough excess fat to achieve what the patient wants, so I will pair fat grafting with implants in a hybrid operation. Butt implants are made from silicone rubber, or elastomer, and can significantly increase size and define shape. The downside is that they tend to be pretty firm. So, after I insert the implants into each buttock, I place a ring of fat around them to round and soften the overall effect.
Implants are, of course, an involved procedure with a more extensive list of side effects and a longer recovery period than fat grafting. While there s no need to avoid the procedure because of this, the patient should be aware of what he or she is getting into. Fat grafting usually requires a five-day recovery period, while implants extend that to around ten days. Butt implants are unique because the patient is always sitting on them, which means that I see more malposition, serum collections, and hematomas than from implants in other areas. An experienced plastic surgeon can mitigate or avoid these to a significant degree, which is why I always advise a patient to be very selective if he or she is considering having work done in the gluteal area. Ideally, I want to avoid the patient having to go through a second surgery to fix complications from the first it s much better for everyone if it s done correctly the first time.
Male vs Female Bodies
Each gender has an ideal stereotype which drives different procedures. Although I ve performed butt implants on men, it s rare. The ideal male shape calls for a smaller butt with a more accentuated, muscular upper body. While I can t create muscle where there is none, that s rarely what patients want me to do. As men grow older we retain a good bit of our strength, but the aesthetic appearance fades. It can be incredibly discouraging for a man who is lifting weights, putting up good numbers, but not seeing the results he wants in the mirror.
VASER high-definition liposuction is the perfect tool for this. I discuss this in-depth in another chapter, but it s worth mentioning here. The focus areas for men are the pectoralis muscle, the big lats in the back, the serratus along the ribs, the six-pack abdomen, the hips, and the external obliques. HD lipo is a phenomenal tool that works wonders to sharpen and accentuate these areas visually, allowing a man to proudly display the results of all his hard work.
One of the more common procedures I perform on men revolves around gynecomastia, commonly referred to as man boobs. There is a full spectrum here: these can be small breast buds that are just annoying to the weightlifter, or they can be sagging, full-sized breasts that women would envy. Quite a bit of tissue may need to be removed not only fat, but breast glands
